Processo de diagnóstico audiológico de crianças após falha na triagem auditiva neonatal

Resumo: Introduction: The hearing assessment in the first months of life of the child has importance due to the need of early intervention, facilitating the development of the language of those with hearing loss. In this stage, difficulties may occur that lead to evasion or delay in the conclusion of audiological evaluations. Objective: To study the process and audiological diagnosis of children who failed in the Newborn Hearing Screening (NHS). Method: Quantitative, descriptive, observational, retrospective study, with document analysis of the medical records of 51 children, who failed in NHS in maternity hospitals under management of the Municipality of São Paulo, and who were referred to the Centro Audição na Criança - CeAC (DERDIC /PUC-SP) for hearing assessment, between January and June 2021. The children who completed, evaded, or remained in diagnosis were identified. Contact attempts were made by phone or Whatsapp, with the guardians of the children who escaped to identify the reason for the evasion. These data were transcribed in an Excel spreadsheet for descriptive analysis. Results: The rate of attendance at diagnosis was 75% and 25% were found between maternity and referral for assessment at the reference center. Of those who started the assessment, 63.16% finished, half being completed by three months of life. Evasion during the assessment process accounted for 31.58%. The contact attempt was successful in 76% of the children who evaded, identifying the most frequent reasons: child’s illness, distance from home and the center, parents' work. The delay in the conclusion of the assessment was due to aspects related to the child’s health, number of appointments, scheduling logistics, difficulty in natural sleep during evaluations and absences. Conclusion: The data show that the attendance to the hearing assessment was below the recommended, with the evasion between the maternity hospital and the arrival in the assessment and in the assessment process well present in the research, but the contact with these families proved successful. Factors related to the health and natural sleep of the child logistic service and family problems can cause evasion or delay in the assessment conclusion

Nesta etapa pode ocorrer dificuldades que levam à evasão ou à demora na conclusão das avaliações audiológicas. Objetivo: Estudar o processo e diagnóstico audiológico de crianças que falharam na Triagem Auditiva Neonatal (TAN). Método: Estudo quantitativo, descritivo, observacional, retrospectivo, com análise documental dos prontuários de 51 crianças, que falharam na TAN nas maternidades sob gestão da Prefeitura Municipal de São Paulo, e que foram encaminhadas ao Centro Audição na Criança – CeAC (DERDIC /PUC-SP) para diagnóstico audiológico, entre janeiro e junho de 2021. Foram identificadas as crianças que finalizaram, evadiram, ou permanecem em diagnóstico. Tentativas de contato foram realizadas por telefone ou WhatsApp, com os responsáveis das crianças que evadiram, com o objetivo de identificar o motivo da evasão. Estes dados foram transcritos em planilha Excel para análise descritiva. Resultado: A taxa de comparecimento ao diagnóstico foi de 75% e 25% evadiram entre a maternidade e o encaminhamento para o diagnóstico no centro de referência. Daquelas que iniciaram no diagnóstico, 63,16% finalizaram, sendo que metade concluído até os três meses de vida. A evasão durante o processo de diagnóstico contabilizou 31,58%. A tentativa de contato foi bem-sucedida em 76% das crianças que evadiram, identificando os motivos mais frequentes: adoecimento da criança, distância da moradia e o centro, trabalho dos pais. A demora na conclusão do diagnóstico se deu a aspectos relacionados à saúde da criança, número de consultas, logística de agendamento, dificuldade para o sono natural durante as avaliações e as faltas. Conclusão: Os dados mostram que o comparecimento ao diagnóstico ficou abaixo do recomendável, com a evasão entre a maternidade e a chegada no diagnóstico e no processo de diagnóstico bem presentes na pesquisa, porém o contato com essas famílias se mostrou bem-sucedido.
