Cânula nasal de alto fluxo em crianças com bronquiolite: uma revisão sistemática

Resumo: | Introduction: Acute viral bronchiolitis (AVB) is an acute viral infectious syndrome that initially manifests itself in the upper respiratory tract and progresses to manifestations in the lower respiratory tract, damaging the bronchioles. The infection causes inflammation in the mucous membranes lining the distal airways, thus causing edema, congestion and a reduction in the caliber of the bronchioles, making the passage of air impossible and obstructing airflow1. The use of HFNC in AVB results in improved oxygenation and ventilation, and may be an alternative to MV8. This study aims to verify the effectiveness of using high-flow nasal cannula used in the treatment of Bronchiolitis in children. Method: A systematic review of the literature was carried out following the recommendations of the items in the Preferred Report for Systematic Reviews and Meta-Analyses 2015 (PRISMA-P)10. Complete observational (cohort, cross-sectional and prospective) studies published between 2018 and 2023 in the electronic databases MedLine (PubMed Interface), PEDro, Lilacs and SciELO were included. The search was carried out during the period from July 1st to August 31st, 2023. The methodological quality of the studies was evaluated according to the PEDro Scale12. Results: It proved that the decrease (%) in respiratory rate and heart rate compared to baseline was significantly greater in the HFNC group. The workload of the accessory respiratory muscles is also reduced, delaying the progression to respiratory failure and reducing the rate of intubation13. The length of hospital stay was significantly different between children treated with HFNC and children treated with non-invasive mechanical ventilation (NIV) modes nCPAP or BiPAP14. Although most of the studies included in this review point out many benefits related to HFNC when compared to other therapy techniques, there were studies that showed opposite results, treatment using CPAP/NPPV was better in preventing treatment failure and obtained a higher rate successful treatment22. Discussion and Conclusion: In the present study it was found that the use of HFNC for the treatment of Bronchiolitis in children has beneficial effects, however the lack of standardization between methodologies, techniques, parameters and treatment time to be compared created doubts about the best method to use. Standardization in these aspects is necessary so that there is better comparison and more concrete results related to the use of HFNC in the treatment of Bronchiolitis in children |

A infecção provoca inflamações nas mucosas que revestem as vias aéreas distais, assim, ocasionando em edemas, congestão e na redução do calibre dos bronquíolos, impossibilitando a passagem de ar e obstruindo o fluxo aéreo. O uso da CNAF na BVA resulta na melhora da oxigenação e da ventilação, podendo ser uma alternativa à VM. Este estudo tem como objetivo verificar a eficácia do uso da cânula nasal de alto fluxo utilizado no tratamento de Bronquiolite em crianças. Método: Foi realizada uma revisão sistemática da literatura seguindo as recomendações dos itens do Relatório Preferencial para Revisões Sistemáticas e Análises de Meta-Análises 2015 (PRISMA-P). Foram incluídos estudos observacionais (coorte, transversal e prospectivo), completos publicados no período de 2018 a 2023 nas bases de dados eletrônicas MedLine (Interface PubMed), PEDro, Lilacs e SciELO. A busca foi realizada durante o período de 01 de julho a 31 de agosto de 2023. A qualidade metodológica dos estudos foi avaliada de acordo com a Escala PEDro. Resultados: Comprovou que a diminuição (%) na frequência respiratória e na frequência cardíaca em comparação com o valor basal, foi significativamente maior no grupo CNAF. A carga de trabalho dos músculos respiratórios acessórios também é reduzida, retardando a progressão para insuficiência respiratória e diminui a taxa de intubação. O tempo de internação hospitalar foi significativamente diferente entre crianças tratadas com CNAF e crianças tratadas com ventilação mecânica não invasiva (VNI) modos nCPAP ou BiPAP. Apesar de grande parte dos estudos incluídos nesta revisão apontarem muitos benefícios relacionados à CNAF quando comparados a outras técnicas de terapia, houve estudos que apontaram resultados contrários, o tratamento a partir de CPAP/NPPV foi melhor na prevenção da falha do tratamento e obteve maior taxa de sucesso no tratamento. Discussão e Conclusão: No presente estudo constatou-se que a utilização da CNAF para o tratamento de Bronquiolite em crianças, tem efeitos benéficos, porém a falta de padronização entre as metodologias, técnicas, parâmetros e tempo de tratamento a serem comparados criaram dúvidas sobre o melhor método a ser utilizado.
