Resumo: | Voice therapy techniques are used by voice clinics so as to soothe or eliminate vocal complaints. The voiced tongue vibration technique is one of the most used techniques in the voice clinic. Therefore, it is considered a universal technique. However this technique can be not so accurately applied and/or performed by the patient at times. The purpose of this study was to analyze the voiced tongue vibration technique of 20 female speech pathologist with a minimum 5 year voice clinic practice with no vocal complaint or voice disorder. The following parameters were analyzed: (1) clinical practice; (2) vocal tract visual image (postural, facial, and vocal tract structures images during 1minute and half of voiced tongue vibration technique); (3) acoustics outcomes (perceptual and acoustic analysis). The subjects answered 2 different protocols, one regarding professional experience and the other one about specific learning on the voiced tongue vibration technique. Digital recording was performed on the subjects prior to and following 1 minute and half of voiced tongue vibration technique in the speaking fundamental frequency during the fiber optic videolaringoscopy recording. The recording material consisted of: date, sustained vowel /a/ and /e/, days of the week and a sentence from CAPE-V translated phonetically into Portuguese - Minha mãe namorou um anjo . In the same time it was videotaped the posture and faces of the subjects during the technique performance. The study data was analyzed by trained listeners. The data underwent statistic analyses. The results showed that SLP specialized in the voice area tend to have a continuous improvement on the voiced tongue vibration technique through specialization courses, masters, and PhDs. The results also indicated that the SLP first learn the technique in college and that they only affirm to have absolute control on the technique when the patients start to show voice improvement. Most of the SLP believe that the main goal of the technique is to enhance mucosal wave and improve glotic closure. Most SLP select the technique for nodule treatment which can show that the technique selection is still based on the ENT diagnosis and not on the vocal features. The use of voiced tongue vibration technique in speaking fundamental frequency presented no impact on pitch. Results also showed that after the technique performance hard glottal attack was milder and voice quality got worse in some cases, maybe due to mucus enhance caused by the fiber optic laryngoscopy examination. The vocal tract presented anterior-posterior pharyngeal constriction during technique. There was no amplification of the pharynx nor vestibular vocal folds medialization or vibration. Nevertheless, results indicated a whole vibration of the larynx box and lowering of the larynx in some of the subjects, maybe due to a subtle change on the fundamental frequency during the technique. Vocal folds were rarely visualized during the technique due to supraglottic constriction. However 11 subjects presented incomplete vocal fold closure. There is no relation between head/neck/face posture change and the vocal tract behavior. The acoustic analysis showed no change in the fundamental frequency after the technique performance (11 subjects). Data showed that there was enhancement of harmonics and significant improvement on the spectrographic trace. Regarding vocal tract behavior, more specifically to glottic closure, the study concluded that the findings of this research differ from the literature. However the perceptual and acoustic analysis indicated similar results with the literature specially related to noise-to-harmonic ratio |

A vibração de língua é uma das técnicas mais utilizadas na clínica da voz e por ser dita universal, o emprego da técnica pode ocorrer de forma indiscriminada, ou seja, nem sempre bem aplicada pelo profissional ou devidamente executada pelo paciente. O objetivo da pesquisa é analisar a técnica de vibração de língua, sob os aspectos do seu aprendizado, da imagem do trato vocal, dos efeitos acústicos por meio da avaliação perceptivo-auditiva e da análise acústica e da imagem postural e da face do fonoaudiólogo durante a realização da técnica. No método foram selecionados 20 fonoaudiólogos do sexo feminino, atuantes na área de voz, com no mínimo cinco anos de experiência clínica, sem queixas ou alterações vocais que se dispuseram a participar dessa pesquisa. A participação constou do preenchimento de dois protocolos, sendo um sobre sua formação profissional e outro sobre o aprendizado da técnica de vibração de língua. Após essa etapa, os sujeitos tiveram suas vozes gravadas antes e depois de realizar um minuto e meio da técnica em tom habitual de fala. Essa gravação constou da emissão da data de realização do exame, duas vogais sustentadas /a/ e /é/, dias da semana e frase do protocolo CAPE-V Minha mãe namorou um anjo . Em seguida foram encaminhados para a realização de exame nasofibrolaringoscópico no qual as estruturas do trato vocal foram visualizadas enquanto a técnica foi realizada durante um minuto e meio. Ao mesmo tempo em que as estruturas do trato eram filmadas, uma câmera de vídeo foi posicionada na frente dos sujeitos a fim de observar a face e a postura de cabeça dos mesmos. As gravações de áudio e das imagens foram analisadas por juízes e foi feita a análise estatística. Os resultados apontaram que o fonoaudiólogo que atua na área de voz mantém um aprendizado continuado, ou seja, realiza cursos de aprimoramento, especialização, mestrado e doutorado. Sobre o aprendizado da técnica, verificamos que o fonoaudiólogo aprende a realizar a técnica na Faculdade e afirma possuir domínio sobre a mesma quando os pacientes começam a apresentar resultados positivos. A maioria dos fonoaudiólogos acredita que a mobilização da onda mucosa e a melhora da coaptação glótica representam a finalidade da técnica. O fonoaudiólogo aplica a técnica principalmente quando há nódulos vocais, demonstrando que a aplicação da técnica ainda se dá pelo diagnóstico médico e não pelas características vocais do paciente. Sobre os achados da avaliação perceptivo-auditiva verificamos que a vibração de língua, quando realizada no tom habitual da fala não altera o pitch. O ataque vocal apresentou maior suavidade e a qualidade vocal piorou em alguns casos, em função do aumento do muco causado pela introdução do fibroscópio flexível. O trato vocal comportou-se, durante a técnica, com constrição de faringe, principalmente no sentido ântero-posterior. Não houve ampliação da faringe, medialização das pregas vestibulares ou vibração das mesmas. No entanto houve vibração de todo arcabouço laríngeo, com abaixamento da laringe em alguns sujeitos, devido à variação discreta do tom utilizado durante a realização da técnica. As pregas vocais nem sempre foram visualizadas devido à constrição supraglótica. Porém, na maioria dos sujeitos (11), foi possível visualizar fechamento glótico incompleto. Não houve, neste estudo, interferência da mudança da postura de cabeça e das modificações da face nas estruturas do trato vocal. A análise acústica mostrou que a freqüência fundamental não se modificou após a realização da técnica de vibração de língua, em 11 sujeitos. Houve aumento dos harmônicos e melhora significativa do traçado espectrográfico. Concluímos que os achados dessa pesquisa diferem dos achados de literatura no que se refere ao comportamento do trato vocal durante a realização da técnica, principalmente no que se refere ao fechamento glótico.
