Articulações entre linguagem, corpo e psiquismo em crianças surdas em decorrência de meningite bacteriana: estudo de dois casos clínicos

Resumo: Introduction: In this work, the articulation between Speech Teraphy and the Psychoanalysis delimited the inquiry perspective: the relation between language and psyquism in the deaf children who had suffered precocious organic blows. It is in this perspective that the deafness as sequel of the bacterial meningitis was analyzed, since the focus, from the clinical experience, had been the possible effect of such clinical picture in the symbolic functioning of these citizens. Front to the risk and the permanent marks of the illness, as much the corporal project as the unconscious image of the body, of these children, got weak (Dolto, 2004) and, such condition, was incresead by the hospitalization (Golse, 2003). Aim: to analyze the effect of the process of precocious illness, for bacterial meningitis, in the psychic functioning and of verbal language of deaf children Method: research of physician-qualitative nature, developed through the study of two clinical cases, João and Bela, that had respectively been sick by bacterial meningitis with 1:6 and 1 year of ages having, both, acquired bilateral deep deafness as sequel. The interpretation of the clinical material, systemize through written registers and of the memory of the researcher, was based on two axles: the functioning of the language and the psychic functioning. Results: The language disturbs, presented for the participant children of the studies of cases, were not justified, exclusively, for the auditory sensorial deficit. On the basis of the premise of that language and the psyquism is lended affected (Cunha, 2004), were possible to observe that the measure where the language of the children evolves, facilitated subjective constitution to it of the same ones. A new look for these children and family allowed to dislocate them of the place of the illness for the one of interlocutors. When assuming autonomy and breaking of listening and intervention in these two levels these children had improved its familiar, interpersonal relations and the corporal position. Conclusions: The case studies had confirmed the initial proposal of this research; also it was possible to establish the relation between precocious of the illness and aggravation of the psychic prognostic of language and; in both the cases the only organic sequel of the meningitis was the deafness, the symbolic sequels had been the main target of study, this because, in the studied cases, physical and symbolic removals had occurred, between the children and its familiar ones, that, allies the communication of the illness, had generated fight it anticipated in imaginary the familiar one; thus, ahead of a so forceful organic fact, one suggests that the therapists retake, with the patient and its familiar ones, the subjective contents of clinical history since the period of illness; the etiology of the deafness (in the case, the bacterial meningitis) was boarded in its bio-psychic dimension, leaving marks on the language, in the singularity of each particular clinical case

É nessa perspectiva que a surdez como seqüela do acometimento por meningite bacteriana foi analisada, já que o foco, a partir da experiência clínica, foram os possíveis efeitos de tal quadro clínico no funcionamento simbólico desses sujeitos. Frente ao risco e às marcas permanentes da doença, tanto o esquema corporal como a imagem inconsciente do corpo das crianças fragilizam-se (Dolto, 2004) e, tal condição, é potencializada pela hospitalização (Golse, 2003). Objetivo: analisar os efeitos do processo de adoecimento precoce por meningite bacteriana, no funcionamento psíquico e de linguagem oral de crianças surdas Método: pesquisa de natureza clínico-qualitativa, desenvolvida através do estudo de dois casos clínicos, João e Bela, que foram acometidos por meningite bacteriana respectivamente com 1:6 e 1 ano de idades tendo, ambos, adquirido surdez profunda bilateral como seqüela. A interpretação do material clínico, sistematizado através de registros escritos e da memória da pesquisadora, foi baseada em dois eixos: o funcionamento da linguagem e o funcionamento psíquico. Resultados: Os transtornos de linguagem apresentados pelas crianças não se justificavam, exclusivamente, pelo déficit sensorial auditivo. Com base na premissa de que linguagem e o psiquismo afetam-se mutuamente (Cunha, 2004), foi possível observar que, a medida em que a linguagem das crianças evoluía, facilitava-se a constituição subjetiva das mesmas. A re- significação das crianças frente à família enlutada, permitiu deslocá-las do lugar de doentes para o de interlocutores; o que favoreceu a elaboração de conflitos e a redução do sofrimento psíquico de ambos. Conclusões: Os estudos de caso confirmaram a proposição inicial desta pesquisa e também foi possível estabelecer a relação entre precocidade do acometimento e agravamento do prognóstico de linguagem e psíquico. Em ambos os casos a única seqüela orgânica da meningite foi a surdez, contudo as seqüelas simbólicas foram intensas e articuladas aos afastamentos físicos e simbólicos, entre as crianças e seus familiares. Tais afastamentos, aliados a comunicação do diagnóstico da doença, geraram o luto antecipado no imaginário familiar.
