Resumo: This paper reports the experience of a nursing student in the process of preparing the plan of health care of the elderly with Chronic Renal Failure hospitalized. The development of care planning aimed to cover the care to the subject in its entirety. The academic experience with the professional reality strengthens the ability for practical application of theoretical learning methodologies concerning nursing care.

A elaboração do planejamento da assistência buscou abarcar o cuidado ao sujeito na sua integralidade.
