Resumo: | Sleep is necessary for human survival, being essential for physical and mental health maintenance in children and adolescents. However, sleep deprivation and/or poor sleep quality has become something common in contemporary society, affecting even the pediatric population. Sleep disorders in childhood and adolescence are associated with several negative health consequences even for individuals with underlying conditions. On the other hand, it´s reasonable think that the pediatric population with chronic obstructive respiratory diseases could have a poor sleep secondary to its own respiratory disease symptoms. So, there is a bidirectional relationship between sleep and chronic obstructive pulmonary diseases in childhood, although there is a limited evidence for it. As instance, there is a restricted number of studies evaluating sleep quality and/or duration and its impact on these subjects’ quality of life. And this is more restricted in pediatric individuals with severe asthma (SA) and post infectious bronchiolitis obliterans (PIBO). Therefore, the goals of the current study were to describe sleep aspects and quality of life of pediatric patients with chronic obstructive respiratory diseases - as cystic fibrosis (CF), SA and PIBO - and to evaluate if there is a correlation between sleep quality and quality of life of these subjects. Forty-six individuals were included: 30 with CF, 09 with severe asthma and 07 with BOPI. Almost 2/3 of the children and adolescents’ parents scored at least 39 points in SDSC, suggesting they had poor sleep quality. Total higher medians were significantly observed in severe asthmatics individuals (76 points; p= 0.001 in comparison to others). Patients and their guardians scored a median of 77 and 80 points respectively in Peds-QL, being those responsible for CF individuals presented higher values than those in other groups (85 points versus 64 and 65; p 0.014). This finding seems to be independent on socioeconomic status, as there was no difference in have or not health insurance. There was inversely moderate correlation between sleep disorders and quality of life (r= -0.532 for kids answer and - 0.606 for parents respectively, with p< 0.001). Therefore, children and adolescents with chronic obstructive respiratory diseases have impairment in their sleep quality and quality of life. Besides, sleep and quality of life have a moderate correlation in these individuals. |

No entanto, a privação de sono e/ou o sono de inadequada qualidade tornou-se algo comum na sociedade contemporânea, acometendo inclusive a população pediátrica. Distúrbios de sono na infância e adolescência estão associados a diversas consequências negativas à saúde, não só a de indivíduos hígidos, mas também a daqueles portadores de comorbidades. Ainda, a população pediátrica com doenças respiratórias obstrutivas crônicas está sujeita a ter um impacto negativo em seu sono, que pode ser potencialmente verificado através de uma piora na qualidade, devido aos sintomas decorrentes da comorbidade de base. Embora haja alguma evidência na literatura para associação entre os distúrbios e/ou a qualidade do sono com o controle de doenças respiratórias obstrutivas crônicas na infância, há uma série de limitações na evidência: restrita quantidade de trabalhos que avalia a duração do sono e o impacto na qualidade de vida destes sujeitos; pequena quantidade de estudos avaliando aspectos do sono em indivíduos pediátricos com asma grave e bronquiolite obliterante pós-infecciosa. Além disso, a evidência existente é muitas vezes heterogênea e até de difícil possibilidade de associação de dados para, por exemplo, efetuar-se um estudo de revisão. Sendo assim, os principais objetivos deste estudo foram descrever os aspectos do sono e a qualidade de vida de pacientes pediátricos com fibrose cística, asma grave e bronquiolite obliterante pós-infecciosa e avaliar a correlação entre a qualidade do sono e a qualidade de vida, estimada pelos próprios indivíduos e por seus responsáveis. Foram incluídos quarenta e seis crianças e adolescentes: 30 portadoras de fibrose cística; 09, de asma grave; e 07, com bronquiolite obliterante. Quase 2/3 dos pais das crianças e adolescentes pontuaram pelo menos 39 pontos na Escala de Distúrbios do Sono para Crianças, indicando que estes jovens tinham má qualidade de sono. Foram observados valores totais mais elevados nos asmáticos graves (76 pontos; p= 0,001) em comparação com a estimativa dos demais grupos de doenças para este questionário. Ainda, os pacientes e seus responsáveis obtiveram mediana de 77 e 80 pontos respectivamente no Questionário Pediátrico de Qualidade de Vida, apresentando os pais daqueles com fibrose cística valores superiores aos daqueles dos demais grupos (85 pontos versus 64 e 65; p= 0,014). Este achado não parece ser dependente do status socioeconômico, uma vez que não houve diferença no fato de possuir ou não plano de saúde. Além disso, houve correlação moderada e inversamente proporcional entre distúrbios do sono e qualidade de vida (r de -0,532 para resposta das crianças e de - 0,606 para os pais, respectivamente, com p< 0,001).
