Resumo: The constant evolution of virtual tools in the field of Dentistry provides professionals with resources to improve diagnosis, treatment planning, and analysis of results. The objective of the present work was to study the new technologies associated with Orthodontics, such as the use of Cone-Beam Computed Tomography (CBCT), digital models, orthodontic aligners, and the Surgery-First aproach. Thus, this study was divided into 3 parts. In the first, a systematized review of the literature was carried out on Surgery-First with orthodontic aligners, in the PubMed, Cochrane, and Embase databases (ARTICLE 1). In the second part of the study, a new protocol was presented, established in the disciplines of Orthodontics and Oral and Maxillofacial Surgery at the School of Health and Life Sciences of the Pontifical Catholic University of Rio Grande do Sul (PUCRS), reporting how the diagnosis and planning should be carried out with digital models and CBCT for use in Surgery-first approach associated with orthodontic aligners, as a predictable and effective alternative when performed judiciously (ARTICLE 2). In the third part of the study, the stability of palatal rugae, after maxillary expansion, was evaluated in digital models with 3 different protocols. A total of 110 models of the upper arch were scanned and analyzed, from 55 patients, divided into 4 groups, according to the procedure: Control (without intervention); Rapid maxillary expansion (RME); Expansion and constriction (Alt-Ramec), and Expansion with a removable orthodontic appliance (ROA). The models were obtained at two different timepoints: initial (T0) and 1 year after maxillary expansion or follow-up (T1). Based on the evaluated sample and with this study design, it was concluded that, in the control group, in growing children, without intervention, the most medial point of the first and third rugae showed stability after one year follow-up and can be used as stable references. In the groups undergoing expansion, there seemed to be differences in the response of the palatal rugae, according to the expansion protocol employed. Thus, as long as these differences are not completely clarified, it is not recommended to consider the palatal rugae as stable references, in cases of maxillary expansion.

O objetivo do presente trabalho foi estudar as novas tecnologias associadas à Ortodontia, como uso de tomografias de Cone Beam (TCCB), modelos digitais, alinhadores ortodônticos e cirurgia de benefício antecipado. Assim, este estudo foi dividido em 3 partes. Na primeira, foi realizada uma revisão da literatura, sobre cirurgia de benefício antecipado com alinhadores ortodônticos, nas bases de dados PubMed, Cochrane e Embase. Foram encontrados 4 artigos, sendo 2 relatos de casos e 2 pesquisas. Os estudos incluídos nesta revisão de literatura sugerem a viabilidade da realização de tratamento ortodôntico-cirúrgico com o uso de alinhadores e cirurgia de benefício antecipado. É necessária a realização de estudos clínicos randomizados para melhor análise e comparação entre os métodos de tratamento orto-cirúrgicos “convencional” e de benefício antecipado associado aos alinhadores, já que a literatura apresenta apenas trabalhos com baixo nível de evidência científica.(ARTIGO 1). Na segunda parte do estudo, foi apresentado um novo protocolo, estabelecido nas disciplinas de Ortodontia e Cirurgia Bucomaxilofacial da Escola de Ciências da Saúde e da Vida da Pontifícia Universidade Católica do Rio Grande do Sul (PUCRS), relatando como é realizado o diagnóstico e planejamento com modelos digitais e TCCB para uso em cirurgias de benefício antecipado associadas com alinhadores ortodônticos. A partir dos resultados obtidos nos 10 casos tratados até o momento, é possível concluir que o emprego dos alinhadores ortodônticos na cirurgia de benefício antecipado traz diversas vantagens aos pacientes, além de proporcionar resultados orto-cirúrgicos adequados e estáveis, semelhantes aos atingidos com o método tradicional, quando bem indicados e planejados. (ARTIGO 2) Na terceira parte do estudo foi avaliada, em modelos digitais, a estabilidade das rugas palatinas após a expansão maxilar com 3 protocolos diferentes. Foram escaneados e analisados 110 modelos da arcada superior, de 55 pacientes, divididos em 4 grupos, conforme procedimento: Controle (sem intervenção); Expansão rápida da maxila, (RME); Expansão e constrição (Alt-Ramec) e Expansão com aparelho removível (ROA). Os modelos avaliados foram obtidos em dois momentos: período inicial (T0) e 1 ano após a expansão maxilar ou acompanhamento (T1). Com base na amostra avaliada e no delineamento deste estudo pôde-se concluir que, no grupo controle, com crescimento e sem intervenção, o ponto mais medial da primeira e terceira rugas apresentaram estabilidade no período de 1 ano, e podem ser usadas como pontos de referência estável. Nos grupos submetidos à expansão, parece haver diferenças na resposta das rugas palatinas, de acordo com o protocolo de expansão empregado.
