Associação entre biomarcadores e desfecho clínico em pacientes com COVID-19 admitidos em UTI

Resumo: Context: The COVID-19 pandemic, caused by the severe acute respiratory syndrome virus 2 (SARS-CoV-2), has had major impacts on society and in numerous contexts. Faced with this moment, which challenges modern medicine with the increasing admission of patients to the Intensive Care Unit (ICU) and the increase in mortality, a better understanding of the association of biomarkers associated with the outcome of the disease is necessary. We aimed to analyze the association between biomarkers, from patients infected with SARS-CoV-2 obtained upon admission to the ICU, and the clinical outcome. Materials and methods: In this cross-sectional study, we included 60 patients diagnosed with COVID-19 admitted to the ICU. Demographic data, clinical history and investigation of biomarkers collected at admission were analyzed. Patients were evaluated based on the clinical outcome: hospital discharge or death. Results: The mean age of the hospital discharge group (62.50 ± 12.39) was lower than that of the death group (69.29 ± 12.7). Procalcitonin, creatinine, and urea levels were significantly higher in the death group compared to the hospital discharge group. Albumin and glucose levels were lower in the death group compared to the hospital discharge group (p<0.05). Conclusion: Age and the laboratory biomarkers procalcitonin, urea, creatinine, albumin and glucose obtained from patients infected with SARS-CoV2 on admission to the ICU were significantly associated with clinical outcome. Patients with high levels of procalcitonin have a 3.5 times greater risk of progressing to death and premature outcome compared to patients with normal levels.

Perante esse momento, que desafia a medicina moderna com a crescente admissão de pacientes em Unidade de Terapia Intensiva (UTI) e o aumento de mortalidade, é necessário um melhor entendimento acerca da associação dos biomarcadores associados ao desfecho da doença. Objetivamos analisar a associação entre biomarcadores, de pacientes infectados por SARS-CoV-2 obtidos na admissão em UTI, e o desfecho clínico. Materiais e métodos: Neste estudo transversal incluímos 60 pacientes com diagnóstico de COVID-19 internados em UTI. Dados demográficos, história clínica e investigação de biomarcadores coletados na admissão foram analisados. Os pacientes foram avaliados com base no desfecho clínico: alta hospitalar ou óbito. Resultados: A média de idade do grupo alta hospitalar (62,50 ± 12,39) foi menor do que o grupo óbito (69,29 ± 12,7). Níveis de procalcitonina, creatinina, e ureia foram significativamente maiores no grupo óbito em relação ao grupo alta hospitalar. Níveis de albumina e glicose estavam inferiores no grupo óbito com relação ao grupo alta hospitalar (p<0,05). Conclusão: Idade e os biomarcadores laboratoriais procalcitonina, ureia, creatinina, albumina e glicose, obtidos de pacientes infectados por SARS-CoV2 na admissão em UTI, estavam significativamente associados com desfecho clínico.
