Desenvolvimento e valida??o de um instrumento de rastreamento de sintomas de transtornos alimentares em crian?as e adolescentes

Resumo: Introduction: Eating disorders are characterized by a persistent disturbance in eating or not eating behavior. Today, anorexia nervosa, bulimia nervosa, and binge eating disorder are the most largely literature. Early diagnosis generates a great success rate. However, today there is a shortage of tools that track eating disorders symptoms, particularly in Brazilian preschoolers. The objective of this study is to develop and verify the evidence of validity of an instrument for tracking symptoms of eating disorders in children and teenagers. Methods: Cross-sectional study that included participants between 5 and 12 years old of both sexes. Data collection was carried out for convenience in schools, sports clubs, and on-line. The validation processes included the following evaluations: content validity (quantitative and qualitative analysis of the judges), Item response theory (severity and discrimination of items and test information curve), construct validity (exploratory and confirmatory factor analysis), concurrent validity (variables: age, BMI and sex; KruskalWallis test) and reliability (omega and Spermann-Brown a test). Results: Regarding content validity, the judges answered that they agree with language clarity (83,3%), relevance in practice (75%), and theoretical relevance (91,7%). The instrument presented an adequate performance about severity and discrimination and the theory response's information curve to the item test. The model proposed in the AFE had a total percentage of variation explained was 69%, and the KMO value was 0,42. The factorial loads of some criteria were evaluated in the confirmatory analysis, and the results of the reliability presented values lower than expected. Female children had higher levels of eating disorders than male children and teenagers. Conclusions: The preliminary version of the instrument for assessing the level of eating disorders for children and teenagers developed performed well part of the psychometric evaluated. However, it was possible to diagnose with the other results that some facets for assessing the level of eating disorder were not included in the developed instrument.

A anorexia nervosa, bulimia nervosa e transtorno de compuls?o alimentar, s?o hoje os mais descritos na literatura. O diagn?stico precoce gera uma grande taxa de sucesso no tratamento, por?m, hoje h? uma escassez de ferramentas que rastreiam os sintomas desses transtornos alimentares, particularmente em pr?-escolares brasileiros. O objetivo deste estudo desenvolver e verificar a evid?ncia de validade de um instrumento de rastreamento de sintomas de transtorno alimentar em crian?as e adolescentes. M?todos: Estudo transversal que incluiu participantes com idades entre 5 e 12 anos, de ambos os sexos. A coleta dos dados foi realizada por conveni?ncia em escolas, clubes esportivos e internet. O processo de valida??o contemplou as seguintes avalia??es: validade de conte?do (an?lise quantitativa e qualitativa dos ju?zes), Teoria da resposta ao item (severidade e discrimina??o dos itens e curva de informa??o do teste), validade de construto (an?lise fatorial explorat?ria (AFE) e confirmat?ria (AFC)), validade concorrente (com as vari?veis: idade, IMC e sexo com o teste Kruskal-Wallis) e confiabilidade (teste ?mega e Spermann-Brown). Resultados: Na validade de conte?do, os ju?zes responderam que concordam totalmente com clareza de linguagem (83,3%), pertin?ncia na pr?tica (75%) e relev?ncia te?rica (91,7%). O instrumento apresentou um desempenho adequado em rela??o ? severidade e discrimina??o e ? curva de informa??o do teste da Teoria de resposta ao item. O modelo proposto na AFE teve um percentual total de varia??o explicado foi de 69% e o valor do KMO de 0,42. As cargas fatoriais dos itens, o desempenho de alguns crit?rios avaliados na an?lise confirmat?ria e os resultados da confiabilidade apresentaram valores inferiores ao esperado. As crian?as e os adolescentes do sexo feminino apresentaram n?veis de transtornos alimentares superiores aos do sexo masculino. Conclus?es: A vers?o preliminar do instrumento de avalia??o de transtorno alimentar desenvolvido para crian?a e adolescentes apresentou bom desempenho em parte das propriedades psicom?tricas avaliadas.
