Correlação entre fibromialgia e desordens temporomandibulares : avaliação dos níveis salivares de IL‐1β

Resumo: | This study examined the association between fibromyalgia and temporomandibular disorders (TMD). The relationship between these disorders has been investigated by analysis of the pro-inflammatory cytokine, interleukin-1β (IL-1β) in the saliva of patients with temporomandibular disorders (TMD); fibromyalgia; Fibromyalgia + temporomandibular disorder; individuals without any of the two conditions (control group), correlating the levels of IL-1β with the clinical findings. All the participants of the study were female, aged from 20 to 84 years. The Group A consisted of 14 healthy patients; the Group B consisted of 17 patients with TMD; the Group C consisted of 11 patients with fibromyalgia; and the Group D consisted of 7 patients with TMD and fibromyalgia associated. The research was conducted in the clinical disciplines of Occlusion and Surgery, at the Dental School, and the Rheumatology Service of Hospital São Lucas, of the Pontifical Catholic University of Rio Grande do Sul (PUCRS). The clinical data collection was performed through anamnesis and clinical examination of all groups was performed using the Research Diagnostic Criteria (RDC) for evaluation of TMD, axis I and II for groups B and D; RDC axis I for groups A and D. Saliva samples were collected in a single session, after signing the free and informed consent. After collection, the samples were stored in a freezer at -20 ° C. The concentrations of the inflammatory cytokine IL-1β were evaluated by enzyme assay (ELISA). Data was submitted to descriptive and statistical analyzes. In the four groups the white race was prevalent. Regarding the medical history of the patients in the Group A, bruxism and allergic rhinitis were the most frequent diseases; in the Group B beyond DTM, osteoarthritis and hypertension (HA) were the most prevalent; in the Group C, in addition to fibromyalgia, HA prevailed; in the group D, besides FM and TMD, the pathologies presented were different, without homogeneity. The drugs most used by the participants of the study were: contraceptives for Group A; antihypertensive drugs, antidepressants, and muscle relaxants for Group B; analgesics, anti-inflammatory drugs, antihypertensive agents and antidepressants for the Group C; antidepressants and analgesics for the Group D. The results revealed that there were no differences among the mouth opening degree throughout the four groups, and in all prevailed mouth opening ≥ 35 mm, which is considered satisfactory. The levels of IL-1β were significantly higher in the Groups B, C and D, when compared to the Group A. When analyzed individually, the use of contraceptives may also have influenced the levels of IL-1β. The other variables did not show any influence on IL-1β levels. The completion of this study shows: (i) the existence of a relationship between the investigated diseases and the increase in salivary concentrations of IL-1β; (ii) there was no correlation between the increase in IL-1β and mouth opening; (iii) other studies need to be carried out by increasing the experimental sample and by evaluating IL-1β levels in patients without fibromyalgia or TMD, aged between 40 and 60 years. |

A relação entre estas duas patologias foi investigada por meio da análise da citocina pró-inflamatória, interleucina-1β (IL-1β), na saliva de pacientes apresentando disfunção temporomandibular (DTM); fibromialgia; fibromialgia + disfunção temporomandibular; indivíduos sem nenhuma das duas condições (grupo controle), correlacionando os níveis encontrados com os achados clínicos. Todos os participantes deste estudo eram do sexo feminino, com idade entre 20-84 anos. O Grupo A foi composto por 14 pacientes saudáveis; O grupo B foi composto por 17 pacientes apresentando DTM; O grupo C foi composto por 11 pacientes apresentando fibromialgia; e o grupo D foi composto por sete pacientes apresentando DTM e fibromialgia associadas. A pesquisa foi desenvolvida nas clínicas das disciplinas de Oclusão e Cirurgia da Faculdade de Odontologia da Pontifícia Universidade Católica do Rio Grande do Sul (PUCRS) e no ambulatório de Reumatologia do Hospital São Lucas da PUCRS. A coleta de dados clínicos foi realizada por meio de anamnese e exame clínico de todos os grupos, com aplicação do Research Diagnostic Criteria (RDC) para avaliação de DTM, eixo I e II para os grupos B e D; eixo I do RDC para os grupos A e D. As amostras de saliva foram coletadas em sessão única, após assinatura do termo de consentimento livre e esclarecido. Após a coleta, as amostras foram armazenadas em freezer sob temperatura de -20°C. As concentrações da citocina inflamatória (IL-1β) foram avaliadas por meio de ensaio enzimático (ELISA). Os dados obtidos foram submetidos às análises descritivas e estatísticas. Nos quatro grupos prevaleceu a raça branca. Em relação à história médica das pacientes, no grupo A, bruxismo e rinite alérgica foram as doenças mais encontradas; no grupo B além da DTM, prevaleceram artrose e hipertensão arterial (HA); no grupo C, além da fibromialgia, prevaleceu HA; e no grupo D, além de fibromialgia e DTM, as patologias apresentadas foram distintas, não havendo homogeneidade. Os medicamentos mais utilizados pelas participantes do estudo foram: anticoncepcionais para o grupo A; anti-hipertensivos, antidepressivos, e relaxantes musculares para o grupo B; analgésicos, anti-inflamatórios, anti-hipertensivos e antidepressivos para o grupo C; antidepressivos e analgésicos para o grupo D. Os resultados clínicos revelaram que não houveram diferenças entre o grau de abertura bucal entre os quatro grupos, sendo que em todos, prevaleceu abertura bucal ≥ 35 mm, considerada satisfatória. Já, os resultados da avaliação da citocina pró-inflamatória IL-1β mostraram que as amostras dos grupos caso apresentaram uma concentração significativamente maior dos níveis de IL-1β salivar, em relação ao grupo C; no entanto, entre os grupos caso não houve diferença significante. Quando analisado individualmente, o uso de anticoncepcionais também pode ter influenciado a dosagem de IL-1β. As demais variáveis não parecem ter influência sobre os níveis de IL-1β.
