Fluid therapy in sepsis and the role of echocardiography in evaluating dynamic parameters

Resumo: Internal Medicine physicians work frequently in emergency departments. In this setting we intend to review some controversial features regarding fluid therapy. Although a common practice, its side effects, specific end-points and volume are not well known. Static parameters such as pressure-derived indices are not useful guides to predict the influence of fluids on cardiac output. Dynamic parameters of fluid responsiveness are not well recognized. Investigators who described these dynamic parameters also evaluated the amount of fluids needed to appropriately increase cardiac output, providing a useful guide concerning this particular issue. The usual ways of patient evaluation are not reliable in order to determine whether to opt for fluid therapy or early onset of amine infusion. The authors recommend training in non-invasive techniques such as echocardiography especially for physicians working in emergency medicine.

Sendo uma prática comum, em especial se a hipotensão está presente, os seus efeitos adversos, objectivos e volume adequados são pouco conhecidos. Os parâmetros que habitualmente nos guiam quanto à quantidade de fluidos a administrar não são fidedignos relativamente ao seu efeito no débito cardíaco, pelo que não permitem uma adequada selecção dos doentes que benefi ciam desta terapêutica. Os parâmetros dinâmicos de avaliação do efeito da fluidoterapia são relativamente desconhecidos. A sua melhor compreensão será mais um instrumento na decisão de administrar fluidos.
