Destacou-se também o papel dos médicos no diagnóstico e prevenção dos maus-tratos, incluindo neste campo a importância dos testes de rastreio. Com este trabalho procurou-se disponibilizar possíveis orientações para intervenção num caso de suspeita de abuso. Analisámos ainda quais as motivações que levam atualmente à invisibilidade do problema. Por fim, foi feita uma breve revisão da legislação, nomeadamente portuguesa na tentativa de auxiliar os médicos, na altura de uma tomada de decisão. Reviu-se e analisou-se a literatura médica sobre maus-tratos a idosos, incluindo para tal artigos em língua portuguesa e inglesa, de revistas reconhecidas cientificamente e livros que abordassem o tema em estudo, abrangendo o período de 2009 a 2015, inclusive. As pesquisas foram realizadas na base de dados PubMed e ClinicalKey e no Catálogo da Biblioteca das Ciências da Saúde, com a finalidade de encontrar estudos, artigos de revisão e guidelines acerca do tema maus-tratos a idosos. Foi utilizado para tal, o termo MeSH (Medical Subject Headings) “elder abuse”, que inclui os termos “abuse, elder”; “elder neglect”; “neglect, elder”; “aged abuse” e “abuse, aged”, combinado com os subcabeçalhos “epidemiology”, “therapy”, “prevention and control” e “guidelines” Encontrou-se uma grande falha no que diz respeito à intervenção médica na gestão de uma suspeita de abuso, nomeadamente no diagnóstico e intervenção. Foi revelada a falta de treino médico nesta área e a falta de orientações homogéneas. Nesta revisão foram ainda encontrados vários testes de rastreio que podem ser utilizados para identificar potenciais situações de abuso, nomeadamente o Elder Abuse Suspicion Index, contudo ainda não validado para a população portuguesa. Não houve coerência em relação à obrigatoriedade da denúncia, exceto nos casos de violência considerados crimes públicos e nas situações de perigo iminente, nos restantes deve-se respeitar a vontade do doente. Assim, mais estudos devem ser realizados no sentido de saber quais os conhecimentos dos médicos nesta área, de modo a poder incentivar a uma maior formação destes profissionais, para que sejam capazes de reconhecer potenciais situações de abuso e atuar precocemente. Neste sentido será útil a validação dos questionários de rastreio que poderão facilitar este processo. Ficou ainda claro que é de extrema relevância que mais estudos sejam realizados de modo a colmatar a falta de protocolos homogéneos para rastreio, diagnostico e intervenção numa situação de suspeita de abuso. With the current aging population also increased situations like the mistreatment of elderly. The objective of this work was to define the different types of abuse to which older people can be exposed, its epidemiology and risk factors or protectors. It also highlighted the role of physicians in the diagnosis and prevention of mistreatment, including in this field the importance of screening tests. With this work we tried to provide guidelines for possible intervention in a case of suspected abuse. We also analyzed the motivations that currently lead to the invisibility of the problem. Finally, a brief review of the legislation was made, notably the Portuguese in an attempt to help doctors at the time of decision making. It is reviewed and analyzed the medical literature about the mistreatment of the elderly, including for such articles in Portuguese and English, magazines recognized scientifically and books that addressed the topic under study, covering the period 2009-2015, inclusive. The surveys were conducted in the PubMed database and ClinicalKey and of the Health Sciences Library catalog, in order to find studies, review articles and guidelines about the mistreatment of the elderly subject. Was used for this, the MeSH term (Medical Subject Headings ) “ elder abuse” , which includes the terms “ abuse , elder “ ; “elder neglect “ ; “ neglect, elder “ ; “ aged abuse” and “ abuse , aged “ , combined with the subheadings “ epidemiology” , “ therapy” , “prevention and control” and “ guidelines “. We found a great fault in regard to medical intervention in the management of suspected abuse, particularly in the diagnosis and intervention. It was revealed to lack of medical training in this area and the lack of uniform orientation. In this review we were also found several screening tests that can be used to identify potential cases of abuse, including the Elder Abuse Suspicion Index, however not yet validated for the Portuguese population. There was no consistency in relation to the obligation of the complaint, except in cases of violence considered public crimes and in situations of imminent danger, the remaining must respect the patient’s will. Thus, more studies should be undertaken to find out what the knowledge of physicians in this area, so as to encourage better training of these professionals, so that they are able to recognize potential situations of abuse and act early. In this sense it will be useful for validation of the screening questionnaire which may facilitate this process.
