This is because current intracochlear imaging modalities lack the resolution to answer the underlying cause of deafness. To increase CI surgical outcomes, it is essential to improve visualization of the affected structures by hearing loss - Basilar Membrane (BM) and Tectorial Membrane (TM). Optical Coherence Tomography (OCT) is a diagnostic imaging technology already validated in several areas of medical research, capable of producing harmless high-resolution cross-sectional images. This study proposes the development of an OCT-based probe device capable of Round Window (RW) insertion for 3D reconstruction. The manuscript also tests the feasibility of a manual and an automatic insertion method for intracochlear trauma-less RW inspection. The design and development of the device are discussed first, focusing on the Key Performance Indicators (KPI)s and best tracking methods. The experimental procedure’s second section focuses on the manual device operation mode, discussing the tremor quantification during two ergonomic postures: fixated wrist and fixated elbow. Based on the insertion mode, the tracking absolute error and the hand-pose tremor recorded data, 6 scenarios were classified as traumatic or non-traumatic. The last section tests the developed 3D reconstruction script and the automatic insertion mode of the proposed device in a 3D reconstruction trial. The results showed that the fusionTrack 500 managed to track with an average 85% less error than the Aurora. The results also showed that manual tremor quantification was higher for the fixated elbow and that all six scenarios were rendered non-traumatic. Due to a technical problem with the OCT system, only one trial of the 3D reconstruction was acquired. The reconstruction’s Root Mean Square Error (RMSE) was 0.0239 mm.
