Use of two C-arms for screw fixation in sacral trauma stabilization

Resumo: The use of fluoroscopy equipment - Arc C, by radiology technicians and the interaction with different surgical specialties is an area in expansion and with particularities aligned to the various surgical techniques. The number of equipment, human resources and needs are not always compatible. A technical-radiological case is described of the "use of two C-arms in screw fixation in the stabilization of sacral trauma" and two radiology technicians. The fractures of the sacrum are varied and the option for surgery is in accordance with the type of fracture. The option when it is surgical requires the use of fluoroscopy, usually only one C-arm is chosen, due to the scarcity of equipment and human resources. In this case, two C-arms of the same brand and model were used, a technique already described in the literature, and two radiology technicians.The specificities and positioning of the C-arms, the computed tomography (CT) fracture visualization, the respective inclinations of radiological incidences, the positioning of orthopedic surgeons result in the greater or lesser success of the surgery. Also the synchronization of the two technicians and the technical specificities allow some differentiating result. The choice and adjustment of this solution resulted in a good surgical time, a good set of synchronized imagiological views and at the same time the two incidences, making the radiological practices and the allied C-arms indispensable in the success of the surgery, and confirming that more equipment and more human resources allocated to these procedures are an option to consider.

Nem sempre o número de equipamentos, recursos humanos e as necessidades são compatíveis. Descreve-se um caso técnico-radiológico da “utilização de dois arcos em C na fixação com parafusos na estabilização de trauma do sacro” e dois técnicos de radiologia. As fraturas do sacro são variadas e a opção pela cirurgia está de acordo com o tipo de fratura. A opção quando é cirúrgica obriga à utilização de fluoroscopia, usualmente opta-se apenas por um arco C, pela escassez de equipamentos e recursos humanos. No caso em estudo utilizaram-se dois arcos C da mesma marca e modelo, técnica já descritas na literatura, e dois técnicos de radiologia.As especificidades e posicionamento dos arcos em C, a visualização da fratura em tomografia computorizada (TC), as respetivas inclinações das incidências radiológicas, o posicionamento dos cirurgiões ortopedistas resulta no maior ou menor sucesso da cirurgia. Igualmente a sincronização dos dois técnicos e as especificidades técnicas permitem algum resultado diferenciador.
