Introduction and objectives - The act of observation is a core skill in clinical practice, particularly in the field of neurology. Professor William Campbell explored the concept of Augenblickdiagnose ("diagnosis in the blink of an eye") in his seminal paper, through the narration of clinical vignettes with high-value information supposed to prompt an immediate diagnosis. Our work aims to update Professor William Campbell's seminal paper, by providing new clinical vignettes. Methods - We performed a non-systematic review of the literature and wrote clinical vignettes based on the clinical experience of the authors. Results - We described several clinical vignettes and presented them in separate tables allocated to major fields in neurology. Discussion - Several clinical vignettes regarding classic presentations of neurological conditions are presented. Cognitive biases of this practice are also discussed. Conclusion - The staple of neurology practice resides in a carefully conducted neurological examination and history taking. Clinical vignettes may serve as powerful educational tools.
