Resumo: Objectives: We review seven years of experience of surgical treatment of CD (Caroli Disease) in the HUC (Hospitais da Unversidade de Coimbra). Material and methods: Histopathology records from July 2000 to July 2007 were scanned CD. Patient files were retrieved and reviewed for demographic and clinical data, comorbidity and exam results. Previous diagnosis, medical treatments and other data pertaining to congenital biliary tree malformations were sought. Results: Between July 2000 and July 2007, eight patients with histologically confirmed CD were treated in the HUC. The average age was 41,2 years. Seven patients have a history of previous cholangitis. Six patients had hepatic resection and the remaining two had liver transplantation. At present all eight patients are alive and experience clinical improvement. Conclusion: Hepatic resection is the choice treatment for localized CD, and it is potentially curative. However, for diffuse disease liver transplant may be the only option, and it may provide good long term results. Keywords: Caroli disease; biliar tree dilation; cholangitis

Keywords: Caroli disease; biliar tree dilation; cholangitis Objectivos: Foi revista a experiência de tratamento cirúrgico de DC em 7 anos, nos Hospitais Universidade Coimbra (H.U.C). Material e Métodos: Consultámos os registos dos exames histopatológicos realizados entre Julho de 2000 e Julho de 2007 do Serviço de Anatomia Patológica dos Hospitais da Universidade de Coimbra (H.U.C.). Feita revisão retrospectiva dos processos clínicos, avaliação de dados demográficos e clínicos, patologias associadas, exames analíticos e radiológicos realizados. Foram analisados diagnósticos prévios, tratamentos médicos efectuados, assim como outras patologias consequentes de malformações congénitas dos ductos biliares. Resultados: No período estudado foram operados 8 doentes com DC nos Serviços H.U.C.. Todos eles com diagnóstico confirmado por histopatologia. Tinham idade média 41,2 anos. Apenas um doente não tinha história de colangite prévia. Foram realizadas 6 ressecções hepáticas e 2 transplantes hepáticos. Todos os doentes estão vivos, clínica e analiticamente bem. Conclusão Ressecção hepática para DC localizada é tratamento de eleição com cura dos doentes. Para a doença difusa, a transplantação hepática é a única opção terapêutica válida com bons resultados a longo prazo.
