Resumo: | o create a colonic reservoir for low colo-rectal anastomosis two techniques have been introduced, the Colonic J pouch and the Coloplasty. These techniques either show technical difficulties or a significant number of dehiscences. Therefore a novel reservoir technique whitout such problems would be an important improvement. The experimental study here reported compares the techniques of anastomosis whitout reservoir, with Coloplasty, with J pouch and with the novel technique of Miotomy. To determine the efficacy of these procedures the functions Volume / Diameter, Volume / intra-luminal Pressure, Compliance and maximum bursting volume were studied. The function injected Volume / mean Diameter showed that the J pouch had always larger diameters than the other techniques. No statistical difference was found for the function injected Volume / mean intra-luminal Pressure between the Miotomy technique and the other procedures. The Miotomy and J pouch groups showed a similar Compliance that was greater than what was observed in the Control and Coloplasty groups. The Miotomy technique is a potentially superior alternative for the construction of a colonic reservoir and should thus be evaluated in a human study. Keywords: colonic reservoir; protectomy; colo-rectal anastomosis Duas técnicas têm sido propostas para criação de um reservatório fecal a montante das anastomoses rectais baixas, a construção de uma Bolsa cólica em J e ou de uma Coloplastia. Estas colocam dificuldades anatómicas e / ou apresentam um número significativo de deiscências. Assim, há lugar ao desenvolvimento de novas técnicas que possibilitem a construção de um reservatório que não tenha estes problemas. O estudo aqui reportado foi realizado no animal de experiência e comparou as técnicas de anastomose sem reservatório, com Coloplastia, com Bolsa em J e com a nova técnica de Miotomia. Para determinar a eficácia destes procedimentos analisaram-se as funções Volume injectado / Diâmetro cólico, Volume injectado / Pressão intra-luminal, “Compliance” e o Volume máximo de rebentamento. A função Volume injectado / diâmetro médio revelou que a Bolsa-J apresentou sempre diâmetros maiores que as outras técnicas. A função Volume injectado / pressão intra-luminal média não revelou diferença estatística entre a Miotomia com as restantes técnicas. Na Miotomia, a “Compliance” foi superior à dos grupos Controlo e Coloplastia e semelhante à da Bolsa-J. A técnica de Miotomia é uma alternativa para a construção de reservatório cólicos potencialmente superior às técnicas actualmente empregues, devendo ser estudada no humano.
