A Rela??o Psicoterap?utica ? Dist?ncia: alian?a terap?utica, sentimentos contratransferenciais e carater?sticas dos psicoterapeutas em telepsicologia

Resumo: | No campo da sa?de mental, o interesse sobre terapia ? dist?ncia (telepsicologia) tem aumentado exponencialmente, principalmente com a eclos?o do surto do Sars-Cov-2 que atingiu propor??es pand?micas. Tanto em teoria como em pr?tica, este setting psicoterap?utico parece ser vi?vel e eficaz. No entanto, a maioria dos estudos focam-se nos pacientes, discriminando a perce??o dos cl?nicos em rela??o a vari?veis terap?uticas comuns a diferentes modelos te?ricos, como a alian?a terap?utica e a contratransfer?ncia. Este estudo destinou-se, precisamente, a avaliar a intera??o din?mica entre a alian?a terap?utica e os sentimentos contratransferenciais em cl?nicos experimentados de diversa orienta??o te?rica, com diferente experi?ncia no uso das ferramentas da terapia ? dist?ncia, principalmente antes e depois da pandemia. A amostra foi constitu?da por 34 psic?logos e m?dicos portugueses (24 do sexo feminino e 10 do sexo masculino) com idades compreendidas entre os 28 e os 73 anos de idade com diferente experi?ncia cl?nica. Os participantes preencheram um question?rio online com 3 instrumentos confi?veis de autorresposta que avaliam a identidade psicoterap?utica (ThId), alian?a terap?utica (IAT-R), sentimentos contratransferenciais (LS) e um question?rio sobre a experi?ncia com telepsicologia. Os dados indicam que n?o existem diferen?as na alian?a terap?utica e nos sentimentos contratransferenciais entre terapeutas que faziam uso da telepsicologia antes da pandemia, comparativamente aos que come?aram a usar este setting depois da pandemia. Os meios de comunica??o usados em telepsicologia n?o dependem de nenhum fator da identidade psicoterap?utica. Psic?logos que utilizam a chamada telef?nica, como meio principal de telepsicologia, pontuam significativamente menos na subescala que avalia a vincula??o e s?o prop?cios a sentir-se aborrecidos/as e paralisados/as com os seus pacientes, comparativamente aos que usam videochamada. A alian?a terap?utica global est? negativamente correlacionada com a psican?lise focada na rela??o objetal. O sentimento caloroso/a est? positivamente correlacionado com psicoterapias experienciais e o sentimento sereno/a com a orienta??o centrada no cliente. Por ?ltimo, observou-se que os sentimentos paralisado/a e aborrecido/a est?o ambos positivamente correlacionados com a terapia comportamental. Apesar da amostra reduzida, conclui-se que o principal interesse para projetos de investiga??o futuros dever? ser a identifica??o das carater?sticas ?nicas da telepsicologia por chamada telef?nica na vincula??o e identificar as vari?veis respons?veis pelo aumento dos sentimentos paralisado/a e aborrecido/a em terapia comportamental. / In the field of mental health, the interest in distance therapy (telepsychology) has increased exponentially, especially with the outbreak of Sars-Cov-2 that has reached pandemic proportions. Both in theory and practice, this psychotherapeutic setting appears to be viable and effective. However, most studies focus only on patients, discriminating the perception of psychologists about common variables to different theoretical models, such as the therapeutic alliance and countertransference. Therefore, this study intends to explore the dynamic interaction between the therapeutic alliance and countertransference feelings in experienced clinicians of different theoretical orientation and with different experience in the use of telepsychology tools, especially before and after the pandemic. The sample consists of 34 Portuguese psychologists (24 female and 10 male) aged between 28 and 73 years old with different clinical experience. Participants completed an online questionnaire with 3 reliable self-response instruments that assess psychotherapeutic identity (ThId), therapeutic alliance (IAT-R), countertransference feelings (LS) and a questionnaire about the experience with telepsychology. The data suggests that there are no differences in therapeutic alliance or countertransference among therapists who used telepsychology before the pandemic compared to those who started using this setting after the pandemic. The means of communication used in telepsychology do not depend on any factor of psychotherapeutic identity. Psychologists who use telephone as the main mean of telepsychology, score significantly less on attachment and are prone to feeling ?bored? and ?paralyzed? towards patients compared to those who use video calling. The global therapeutic alliance is negatively correlated with psychoanalysis focused on the object relationship. The warm feeling is positively correlated with experiential psychotherapies and the serene feeling with the client-centered orientation. Finally, it was observed that the paralyzed and bored feelings are both positively correlated with behavioral therapy. Despite the small sample, it is concluded that the main key for future investigations is to identify the unique characteristics of telepsychology by telephone in the bond with patients and to identify which variables are responsible for the increase in paralyzed and bored feelings in behavioral therapy. |
