Atualmente, a dilatação fluxo-mediada (FMD) é o método gold-standard para avaliação da função endotelial. No entanto, dada a sua baixa precisão e especificidade, bem como à sua complexidade, por ser tecnicamente exigente, dependente do operador e requerer um alto nível de treino por parte do mesmo, foi proposto um novo método para avaliação da função vasomotora- a desaceleração fluxo-mediada (FMS), definida como a diminuição da velocidade da onda de pulso após hiperemia reativa, induzida através da colocação de uma braçadeira no antebraço e insuflada por um período máximo de 5 minutos. Embora o FMD e o FMS tenham sido estudados anteriormente em repouso, não é claro o efeito do exercício agudo na função endotelial. Por outro lado, os resultados são divergentes e os estudos escassos. O objetivo deste estudo foi 1) avaliar e comparar a confiabilidade e repetibilidade do FMS e FMD em jovens saudáveis do sexo masculino, em repouso e 2) examinar os efeitos de uma sessão de exercício contínuo de intensidade moderada (MICE) no FMS e FMD. Métodos: Doze participantes saudáveis do sexo masculino, com idade inferior a 40 anos, foram avaliados em dois dias diferentes. Após medições do FMD e do FMS em repouso, os participantes realizaram uma sessão de MICE em passadeira, a uma intensidade de 60 % da frequência cardíaca de reserva (HRR). O FMD e o FMS foram avaliados em três momentos distintos - pré e pós 10 e 60 minutos, para o FMD, e um, dois e três minutos após a hiperemia reativa, para o FMS, pré e pós 10 e 60 minutos. A resposta da velocidade da onda de pulso carótida-radial (cr-PWV) à hiperemia reativa foi quantificada usando o método de tonometria de aplanação, com recurso à colocação de dois mecanotransdutores nas artérias carótidas e radial. A reprodutibilidade e repetibilidade do FMD e FMS, antes e após o exercício, foi avaliada através (1) da análise de variância bifactorial (ANOVA), (2) do coeficiente de correlação intraclasse (ICC), (3) do coeficiente de variação e (4) do gráfico de Bland-Altman, para cada período de tempo. Resultados: O menor coeficiente de variação observado para o FMD nas avaliações de controlo foi de 26% enquanto para o FMS foi de 46%. Após a sessão de exercício, verificaram-se coeficiente de variação superiores a 35% e 50% para o FMD e FMS, respetivamente. O FMD e o FMS demonstraram bom a excelente (ICC ³ 0.74) e mau a bom (ICC ³ 0.07) nível de confiabilidade, respetivamente, para as avaliações de controlo, e mau a moderado (ICC ³ -0.09) nível de confiabilidade, para as avaliações após MICE. Os gráficos de Bland-Altman demonstraram uma fraca concordância entre as medidas, para cada ponto de tempo de condição de controlo e entre as condições. Não se verificaram diferenças estatisticamente significativa no FMD e FMS após o MICE (p > 0.05). Conclusões: O FMS não demonstrou ser uma técnica confiável e repetível, e o MICE não induziu alterações significativas no FMS e FMD.Background: Endothelial function is a predictor of cardiovascular health. Nowadays, flow-mediated dilation (FMD) is the gold-standard method to assess endothelial function. However, because of its poor reliability and specificity, as well as its complexity, because it is technically challenging, operator-dependent and requires a high level of operator training, a new method has been proposed – the flow-mediated slowing (FMS), defined as slowing of the pulse wave velocity after reactive hyperaemia, in conduit arteries. Although the FMD and FMS have been previously studied in rest condition, it is not clear how exercise acutely affects endothelial function, and the results are divergent. We aimed to 1) assess and compare the reliability and repeatability of FMS and FMD in young healthy males at rest and 2) examine the effects of a single bout of a moderate intensity continuous exercise (MICE) in FMS and FMD. Methods: Twelve healthy male participants were assessed on two different days. After preexercise measurements of FMD and FMS, participants undertook an acute bout of MICE on a treadmill, at 60% of the heart rate reserve (HRR). FMD and FMS were assessed in three different moments – pre-and post-10 and 60 minutes, for the FMD, and after 1, 2 and 3 minutes after de reactive hyperaemia, for the FMS, pre-and post-10 and 60 minutes. Carotid-radial pulse wave velocity (cr-PWV) response to reactive hyperaemia was quantified by applanation tonometry. Reproducibility and repeatability of FMD and FMS following exercise were assessed utilizing (1) two-way analysis of variance (ANOVA), (2) intraclass correlation coefficients (ICC), (3) coefficient of variation, and (4) Bland-Altman Plot, each time-period. Results: The lowest coefficient of variation observed for the FMD in the control assessments was 26% while for the FMS it was 46%. After the exercise session, there were coefficients of variation greater than 35% and 50% for the FMD and FMS, respectively. The FMD and FMS demonstrated good to excellent (ICC ³ 0.74) and poor to good (ICC ³ 0.07) level of reliability, respectively, for the control assessments and, poor to moderate (ICC ³ -0.09) level of reliability, for evaluations after MICE. The Bland-Altman plots showed a weak agreement between the measures, for each time point of control condition and between the conditions. There were no statistically significant differences in the FMD and FMS after the MICE (p> 0.05).
