Effectiveness and Predictability of Clear Aligner Appliances Regarding Canine De-rotation: Observational longitudinal retrospective study with integrative systematic review

Resumo: Introduction: Invisalign® aligners have evolved in terms of materials and strategies, increasing their effectiveness. Made of polyurethane and with innovative technology, they promise to treat complex tooth rotations: one of the key factors in orthodontic treatment, having a functional and aesthetic impact on patients. Objectives: To evaluate the effectiveness and predictability of canine de-rotation in the Invisalign® system, to compare de-rotation between refinements and the influence of attachemnts on treatment. Material and methods: A retrospective longitudinal observational study with a sample of 49 patients. The inclusion criteria were: permanent dentition, individuals who had already fully completed orthodontic treatment with aligners without mismatches and planning above 5º of rotation. The movements planned and obtained were taken from the Invisalign® Doctor Site. The IBM® SPSS® statistical method was used for statistical purposes. Results: We found statistically significant differences between predicted and actual rotation. We suggest that there is effective rotation between refinements, up to the second, and that the effectiveness of this movement is always significantly higher with the use of optimised attachments. Discussion: Our results corroborate most studies in the literature: the efficacy and predictability of de-rotating canine teeth is low, which can be explained, among other factors, by their anatomy. In general, both in our study and in the literature, the use of optimised attachments is valued. Conclusions: The low efficiency and unpredictability between planned and achieved values in the canine de-rotation movement and the positive influence of using optimised attachemnts are highlighted.

Feitos de poliuretano e com tecnologia inovadora prometem tratar rotações dentárias complexas: um dos fatores chave no tratamento ortodôntico, tendo impacto funcional e estético nos pacientes. Objetivos: Avaliar a eficácia e previsibilidade da desrotação dos caninos no sistema Invisalign®, comparar a desrotação entre refinamentos e a influência do uso de attachemnts no tratamento. Material e métodos: Estudo observacional longitudinal retrospetivo com uma amostra de 49 pacientes. Os critérios de inclusão foram: dentição definitiva, indivíduos que já tenham concluído, integralmente, o tratamento ortodôntico com alinhadores sem desajustes e planeamento acima de 5º de rotação. Os movimentos planeados e realmente obtidos foram extraídos do Invisalign® Doctor Site. O método estatístico IBM® SPSS® foi utilizado para fins estatísticos. Resultados: Encontramos diferenças estatisticamente significativas entre a rotação prevista e efetiva. Sugerimos que há rotação efetiva realizada entre refinamentos, até ao segundo e ainda que, a eficácia deste movimento é sempre significativamente superior com o uso dos attachments do tipo otimizado. Discussão: Os nossos resultados corroboram a maior parte dos estudos da literatura: a eficácia e previsibilidade da desrotação dos dentes caninos é baixa podendo ser justificado, entre outros fatores, pela sua anatomia. Em geral, tanto no nosso estudo como na literatura, o uso de attachments otimizados é valorizado.
