The genetic diversity of Mycobacterium tuberculosis and its contribution to tuberculosis

Resumo: Introduction: Tuberculosis remains a global problem, with over 10.6 million new cases and more than 1.3 million fatalities each year. Despite a steady decline in these numbers over the past decades, the COVID-19 pandemic has reversed this downward trend. It is globally accepted that the fight against tuberculosis will require better means of prevention, diagnosis, and therapy. Fundamental research in these areas, particularly the understanding of the immune response against Mycobacterium tuberculosis, is essential. Many studies highlight the central role of the immune response in controlling M. tuberculosis infection, also showing that failures in this response often lead to progression to active disease. Although these studies traditionally focus on host characteristics, recent data point to an important role of pathogen genetic diversity in shaping the immune response. This variable has long been neglected due to the low genetic diversity of M. tuberculosis. Objective: With the advent of complete genome sequencing, we realized that, despite modest, the genetic diversity of the pathogen is not negligible. In this publication, we aim to describe how this actively contributes to the outcome of M. tuberculosis infection. Materials and methods: In this article, we present a literature review based on articles available on PubMed and relevant World Health Organization reports on the subject. Results: From the analysis conducted, we present the main arguments demonstrating the impact of M. tuberculosis genetic diversity on various aspects of tuberculosis and discuss how its incorporation into research can contribute to the design of more effective tools against this disease. Conclusion: The differences associated with various M. tuberculosis lineages highlight new challenges for the study of tuberculosis with implications for its prevention, diagnosis, and treatment.

Malgré une baisse constante de ces chiffres au cours des dernières décennies, la pandémie de COVID-19 a inversé cette tendance à la baisse. Il est mondialement acquis que la lutte contre la tuberculose nécessitera de meilleurs moyens de prévention, de diagnostic et de thérapie. La recherche fondamentale dans ces domaines, en particulier la compréhension de la réponse immunitaire contre Mycobacterium tuberculosis, est par consequent essentielle. De nombreuses études soulignent le rôle central de la réponse immunitaire dans le contrôle de l’infection par M. tuberculosis, montrant également que des défaillances dans cette réponse conduisent souvent à la progression vers une maladie active. Bien que ces études se concentrent traditionnellement sur les caractéristiques de l’hôte, des données récentes indiquent un rôle important de la diversité génétique du pathogène dans la formation de la réponse immunitaire. Cette variable a longtemps été négligée en raison de la faible diversité génétique de M. tuberculosis. Objectif: Avec l’avènement du séquençage complet des génomes, on a réalisé que, bien que minime, la diversité génétique du pathogène n’est pas négligeable. Dans cette publication, nous avons l’intention de décrire comment cela contribue activement au résultat de l’infection par M. tuberculosis. Matériaux et méthodes: Dans cet article, nous présentons une revue de la littérature basée sur des articles disponibles sur PubMed et des rapports pertinents de l’Organisation mondiale de la santé sur le sujet. Résultats: De l’analyse réalisée, nous présentons les principaux arguments démontrant l’impact de la diversité génétique de M. tuberculosis sur divers aspects de la tuberculose et discutons de la manière dont son incorporation dans la recherche peut contribuer à la conception d’outils plus efficaces contre cette maladie. Conclusion: Les différences associées aux diverses lignées de M. tuberculosis soulignent de nouveaux défis pour l’étude de la tuberculose avec des implications pour sa prévention, son diagnostic et son traitement.Introdução: A tuberculose mantém-se um problema global, com mais de 10.6 milhões de novos casos e mais de 1.3 milhões de fatalidades por ano. Apesar de um declínio constante destes números ao longo das últimas décadas, a pandemia de COVID-19 reverteu esta tendência decrescente. É globalmente aceite que a luta contra a tuberculose passará por melhores meios de prevenção, diagnóstico e terapia. A investigação fundamental nestas áreas, e em particular a compreensão da resposta imunológica contra o Mycobacterium tuberculosis, é, pois, essencial. Muitos estudos mostram o papel central da resposta imunológica para o controlo da infeção por M. tuberculosis, evidenciando também que falhas nesta resposta conduzem frequentemente à progressão para uma doença ativa. Apesar de classicamente estes estudos serem focados nas características do hospedeiro, dados recentes apontam para um papel importante da diversidade genética do patógeno no moldar da resposta imunológica. Esta variável foi durante muito tempo negligenciada, dada a baixa diversidade genética do M. tuberculosis. Objetivo: Com o advento da sequenciação de genomas completos, percebeu-se que apesar de diminuta, a diversidade genética do patógeno não é negligenciável. Nesta publicação pretendemos descrever como esta diversidade contribui ativamente para o resultado da infeção por M. tuberculosis. Materiais e métodos: Neste artigo apresentamos uma revisão da literatura, baseada em artigos disponibilizados no PubMed e relatórios da Organização Mundial de Saúde pertinentes ao tema. Resultados: Da análise efetuada apresentamos os principais argumentos que demonstram o impacto da diversidade genética do M. tuberculosis em vários aspetos da tuberculose e discutimos como a sua incorporação na investigação pode contribuir para o desenho de novas ferramentas mais eficazes contra esta doença.
