Resumo: | Systemic lupus erythematosus is a chronic inflammatory disease, resulting from an auto-immune dysfunction. The etiology of this diseaseis unknown. It frequently occurs in women of childbearing age. Pregnancy in patients with systemic lupus erythematosus may beassociated with several complications (maternal, obstetrical and fetal). The prognosis for both mother and child is better when systemiclupus erythematosus has been quiescent for at least six months before pregnancy. Thus, preconceptional assessment and managementis crucial for helping women to achieve a period of disease remission before pregnancy as well as for allowing an adjustment oftherapy. Maternal health and fetal development should be closely monitored during pregnancy. These patients should be surveilled by amultidisciplinary team (obstetrician, rheumatologist or internist, nephrologist if necessary and a pediatrician), in a tertiary care hospital.Antiphospholipid syndrome, positivity for anti-SSA/Ro or anti-SSB/LA antibodies, hypertension or renal involvement are associatedwith an increase of adverse pregnancy outcomes. In this article the authors review the main aspects of Systemic lupus erythematosus(SLE) and pregnancy. |

A suaetiologia é desconhecida. O Lupus eritematoso sistémico ocorre frequentemente em mulheres em idade reprodutiva e a sua associaçãocom a gravidez pode colocar problemas maternos (relacionados com a doença de base), obstétricos e fetais. O prognóstico, quermaterno quer fetal, é melhor quando a doença se encontra em remissão ou quiescente, pelo menos nos seis meses anteriores à concepção.Desta forma, o aconselhamento pré-concepcional é fundamental, uma vez que permite não só programar a gravidez para queesta ocorra com a doença em remissão, como também optimizar a terapêutica instituída. Comparativamente a uma gestação de baixorisco, a gravidez numa doente afectada com Lupus eritematoso sistémico obriga a uma maior vigilância obstétrica e do bem-estar fetal.Esta vigilância deve ser efectuada por uma equipa multidisciplinar que englobe obstetra, reumatologista ou internista, nefrologista(se necessário) e pediatra, num hospital de cuidados terciários. Algumas situações clínicas merecem particular atenção, uma vez quecondicionam um maior risco de desfechos adversos durante a gravidez. São exemplos, o síndrome de anticorpos anti-fosfolipídicos, apresença de anticorpos anti-SSA/Ro ou anti-SSB/La, a existência de hipertensão arterial ou de envolvimento renal.
