Os cuidados paliativos no âmbito dos cuidados de saúde primários: as intervenções dos enfermeiros*

Resumo: Since the Reform of the Primary Health Care is being carried out and it is expected that the Palliative Care Units, level I and II, will soon be put into effect, it has become pertinent to understand how the nurses of the Primary Health Care respond to the needs of someone who is in intense suffering near the end of their life, so that the existent resources can be better adapted to the needs and so that the quality of the response might also be improved. As a contribution to attain this goal a semi-structured, exploratory-descriptive investigation with a qualitative approach was planed using the interview as a technique of data gathering. From among the different objectives that guided this study, we focused on the palliative actions that the referred nurses provide to the clients according to the conditions referred to and also on the analysis of their views concerning the care provided and the results that were obtained. According to these results we can assume that, in the present situation, the Health Center to which these nurses belong does not offer the required resources for them to respond in a thorough and adequate way to the multiple and complex needs they are faced with.

According to these results we can assume that, in the present situation, the Health Center to which these nurses belong does not offer the required resources for them to respond in a thorough and adequate way to the multiple and complex needs they are faced with.Estando em curso a Reforma dos Cuidados de Saúde Primários e anunciando-se para breve a entrada em funcionamento de Unidades de Cuidados Paliativos de Nível I e II, tornou-se pertinente compreender como é que os enfermeiros dos Cuidados de Saúde Primários respondem às necessidades da pessoa em sofrimento intenso na fase final de vida, no sentido que daí possa resultar não só uma melhor adequação dos recursos às necessidades, mas também um incremento positivo na qualidade das respostas. Como contribuição para atingir essa meta, foi projectada uma investigação de carácter exploratório - descritivo, com uma abordagem qualitativa, usando como técnica de recolha de dados a entrevista semi-estruturada. Neste artigo, de entre os diferentes objectivos específicos orientadores do respectivo estudo, restringimo-nos à identificação das acções paliativas que os referidos enfermeiros prestam aos utentes nas condições indicadas e à análise das suas percepções relativas aos cuidados prestados e aos resultados obtidos nesse âmbito. Dos achados do estudo depreende-se que, na situação actual, o Centro de Saúde a que pertencem os enfermeiros envolvidos no estudo não lhes possibilita responderem de forma adequada e completa à multiplicidade e complexidade das necessidades com que se deparam.   * Este artigo retoma parte da tese de mestrado da 1.ª autora, apresentada no Instituto de Ciências da Saúde da Universidade Católica Portuguesa – Porto e orientada pela outra autora.
