The dignified death of an “abortion” – bioethical discussion based on a clinical case

Resumo: Introduction: The prevalence of preterm birth has been increasing. As scientific and technological knowledge advances and the limits of viability expand, ethical questions and challenges arise for health professionals and parents. Aim: To promote an ethical reflection on issues and dilemmas involving parents and newborns near to the limit of viability. Materials and methods: Based on a clinical case, the authors intend to reflect on the literature and ethical principles. Discussion: The reflection of the clinical case allowed identifying the ethical principles of beneficence, non-maleficence, autonomy, dignity, vulnerability, double effect and justice. The newborn does not have autonomy, and the parents assume this role during the decision – making process, which must be shared with the healthcare professionals involved. Conclusion: Prematurity raises complex ethical issues, which intersect with parental beliefs and values. Each premature infant is unique and it is impossible to accurately predict the course and prognosis of each one. The ethical problems in prematurity are based on the ultimate value of life, but consolidated on the principles of non-maleficence and beneficence. The discussion of these very sensitive themes must be pondered and reflected.

À medida que os conheci-mentos científicos e tecnológicos avançam e os limites da viabilidade se expandem, colocam-se questões e desafios éticos tanto aos profissionais de saúde como aos pais destas crianças. Objetivo: Abordar e promover uma reflexão ética sobre as questões e dilemas envolvendo os pais e recém-nascidos no limiar da viabilidade.Materiais e métodos: Através da descrição de um caso clínico, pretende-se refletir sobre a literatura e os princípios éticos. Discussão: A reflexão ética deste caso permitiu identificar os princípios éticos da beneficência, não maleficência, autonomia, dignidade, vulnerabilidade, duplo-efeito e justiça. O recém-nascido não tem autonomia, sendo que os pais assumem este papel durante o processo de tomada de decisão, que deve ser partilhado com os profissionais de saúde envolvidos. Conclusão: A prematuridade levanta questões éticas complexas, que se cruzam com as crenças e valores dos pais e familiares destas crianças. Cada recém-nascido prematuro é único, sendo impossível predizer com exatidão o percurso e o prognóstico de cada um. As questões éticas que rodeiam a prematuridade têm por base o valor absoluto do direito à vida, alicerçadas nos princípios da não maleficência e beneficência.
