Resumo: The authors describe a case of multiple system atrophy in a 54 year-old man, with dizziness, syncope and sexual dysfunction as the initial symptoms. Signs of parkinsonism, dysautonomy and cerebellar dysfunction were shown on physical examination and autonomic function tests. Conservative treatment led to clinical improvement. A review of multiple system atrophy was made following this clinical case.

O exame objectivo e testes da função autonómica revelaram sinais de parkinsonismo, disautonomia e disfunção cerebelosa. A terapêutica sintomática permitiu reverter parcialmente as queixas.
