Assessment of primary health care services for patients with suspected dengue in the city of Rio de Janeiro

Resumo: Primary Health Care (PHC) should have physical and organizational conditions to provide adequate care to patients with suspected dengue and prevent death. We aimed to analyze the adequacy of PHC in the city of Rio de Janeiro. It is a normative evaluation designed to assess the structure and process according to the protocols of the Ministry of Health in Brazil. A system of scores was elaborated to assess the APS, according to proposed indicators as follows: adequate (0.8 to 1); partially adequate (0.6 to 0.79); and not adequate (≤ 0.59). As for the structure, the units were partially adequate, with adequate physical and structural conditions, but inadequate regarding standards, technical procedures and number of professionals. The processes were partially adequate, with difficulties related to: implementation of protocols, standardization of assistance flows, inclusion of the multi-professional team and effective communication. In addition, some procedures related to clinical observation, essential for decreasing fatality rate were inadequate. The number of health units has proved to be insufficient, although there has been an increase in APS coverage recently. Management should establish flows to structure and support health care networks with logistical and operational conditions for qualified and effective care.

Management should establish flows to structure and support health care networks with logistical and operational conditions for qualified and effective care. Os Cuidados de Saúde Primários (CSP), denominados “atenção primária à saúde (APS)”no Brasil devem ter condições físicas e organizacionais para oferecer atendimento adequado aos pacientes com suspeita de dengue e evitar óbitos. Objetivamos analisar a adequação da APS no município do Rio de Janeiro. Trata-se de uma avaliação normativa com apreciação da estrutura e processo. Foi elaborado um sistema de escores para analisar a APS, de acordo com os indicadores utilizados: adequada (0,8 a 1); parcialmente adequada (0,6 a 0,79); e não adequada (≤ a 0,59). Quanto à estrutura, as unidades estavam parcialmente adequadas, com condições físicas e estruturais adequadas, porém inadequadas quanto à existência de normas, procedimentos técnicos, além do quantitativo insuficiente de profissionais. Os processos foram parcialmente adequados, com dificuldades apontadas para: implementação de protocolos, uniformização dos fluxos assistenciais, inclusão da equipa multiprofissional e comunicação efetiva. Além disso, alguns procedimentos relacionados à observação clínica, essenciais para a diminuição da letalidade, foram inadequados. Embora nos últimos anos tenha ocorrido aumento da cobertura de unidades de APS, esse não se mostrou suficiente para a ampliação do acesso e atendimento oportuno à população.
