THE HPV PROFILE OF PATIENTS WITH OROPHARYNGEAL TUMORS IN AN ONCOLOGY INSTITUTION IN PORTUGAL BETWEEN 2018 AND 2019 – DESCRIPTIVE RETROSPECTIVE STUDY

Resumo: | Oropharyngeal cancer is a disease with an increasing incidence, mostly detected in advanced stages. This increase in incidence is currently related to Human Papillomavirus (HPV) infection, associated with changes in the sexual behaviour of the population, in the last decades. This study aimed to characterize the oropharyngeal cancer patients’ profile in Northern Portugal regarding the HPV infection status and to analyse the survival profile of those patients. This descriptive retrospective study included 134 patients with histological confirmation of oropharyngeal cancer and immunohistochemical characterization of HPV status, that were referred to the Head and Neck Department of the Instituto Português de Oncologia do Porto, between January 2018 and December 2019. Most of the oropharyngeal cancer cases were diagnosed in male subjects (91,7%). Interestingly, only 14,6% (18/123) of male cases were HPV-positive, in contrast to 63,6% (7/11) HPV-positive tumours diagnosed in female subjects. Moreover, among all patients integrated into this study, only 18,7% had HPV-positive tumours, and the majority of the referred patients (73,9%) were diagnosed at later stages (stage III or IV). In this study, it was also observed that, in later stages, the number of HPV-negative cases increased and the HPV-positive cases decreased. |

Este aumento na incidência é atualmente atribuído à infeção pelo vírus do papiloma humano (HPV), resultante das alterações nos hábitos sexuais da população, nas últimas décadas. O objetivo deste estudo é caracterizar o perfil do paciente com cancro da orofaringe, em Portugal, quanto ao estado positivo ou negativo para infeção por HPV e analisar os perfis de sobrevivência destes doentes. Neste estudo retrospetivo descritivo, foram incluídos 134 doentes com diagnóstico histológico de cancro da orofaringe e com caracterização do estado de HPV por estudo imuno-histoquímico. Todos os doentes foram referenciados à Clínica de Cabeça e Pescoço do Instituto Português de Oncologia do Porto, entre janeiro de 2018 e dezembro de 2019. Dos doentes estudados, apenas 18,7% eram portadores de tumores HPV positivos. A maioria dos tumores (91,7%) foram diagnosticados em indivíduos do sexo masculino. Destes, apenas 18/123 casos (14,6%) eram HPV positivos, em contraste com os 63,6% dos tumores diagnosticados em indivíduos de sexo feminino (7/11 doentes). A maioria dos doentes que deram entrada na instituição foram diagnosticados tardiamente (73,9% eram estádios III ou IV).
