Resumo: It’s past a century since Kraepelin described dementia praecox. This morbid entity was, ten years later, called schizophrenia by Bleuler. Actually is diagnosed according APA or WHO operational criteria. After its first descriptions, many studies investigated the causes, clinical manifestations, course and treatment. Despite the great number of theories about this issue, we think that vulnerability-stress theories are the best approach to account all that is known at the moment. They integrate biopsychosocial and coping factors and give us an intellectual support for treatment and psychosocial rehabilitation work with these patients. DOI: http://dx.doi.org/10.17575/rpsicol.v14i1.517A demência precoce, descrita por Kraepelin há cerca de um século, foi mais tarde designada por esquizofrenia, após a reformulação diagnóstica de Bleuler. Na actualidade o diagnóstico faz-se segundo critérios operacionais recomendados quer pela Associação Americana de Psiquiatria quer pela Organização Mundial de Saúde. Desde a descrição inicial da doença têm sido efectuados inúmeros estudos, com o objectivo de conhecer as suas causas, manifestações e formas de evolução. As teorias da vulnerabilidade-stress são as que melhor integram a pletora de conhecimentos disponíveis sobre esta perturbação, paradigma da psiquiatria e da saúde mental. Servem ainda de fundamento ao tratamento integrado dos doentes, tendo em consideração os diferentes factores biopsicossociais responsáveis pelas recaídas e tipos de evolução clínica. São também um suporte teórico para o trabalho de reabilitação psicossocial dos doentes.
