Expressed emotion of schizophrenic families and the patients' relapse

Resumo: | Conducted a replication study of schizophrenia relapse (12 mo follow-up) and expressed emotion (EE) of patients' families. A sample of 53 chronic schizophrenia families was evaluated with the Camberwell Family Interview. Patients were in psychiatric ambulatory treatment with neuroleptic drugs. After interview analysis, results show that 66% of families had high EE and 34% low EE scores. Hostility was expressed in 15% of relatives and overinvolvement in 23%. 37 patients' relatives were male and 41 female. Female relatives showed more emotional overinvolvement (37%) than male relatives (8%). Follow-up relapse rate was 57% for patients living in high EE families and 17% for patients in low EE families. Comparing data from different studies in different countries, this study, as well as a Spanish one, received less criticism than English studies. In spite of this EE cultural dimension, results agree with the general conclusion that the patient relapse rate is greater in those who live in high than low EE families. Numa amostra de 53 doentes esquizofrénicos crónicos, a EE dos familiares foi avaliada pela Camberwell Family Interview (CFI). Os doentes estavam em tratamento psiquiátrico ambulatório com neurolépticos. A análise de conteúdo das entrevistas revelou que 66% das famílias eram do tipo EE alta e 34% do tipo EE baixa. Os scores das escalas da CFI foram: comentários críticos 4,1 (média), a hostilidade foi expressa por 15% dos familiares e a sobre-implicação emocional por 23%. Trinta e sete familiares são do sexo masculino e 41 do sexo feminino. Os familiares do sexo feminino expressaram mais sobre-implicação emocional (37%) que os do sexo masculino (8%). Durante os 12 meses de seguimento, recaíram 57% dos doentes que viviam em famílias de EE alta e 17% dos que viviam em famílias de EE baixa. A comparação com os resultados de diferentes estudos realizados em diferentes países mostra maior semelhança entre os nossos resultados e os do estudo espanhóis, designadamente em relação ao criticismo dos familiares que é maior nos estudos anglo-saxónicos. Apesar da dimensão cultural da EE, os resultados confirmaram a constatação geral de que recaem mais os doentes que vivem em famílias de EE alta.
