Isolated nasal surgery in the treatment of adult obstructive sleep apnea syndrome – Pilot study

Resumo: Objective: to evaluate the effects of isolated nasal surgery in the treatment of Obstructive Sleep Apnea Syndrome (OSAS), through the impact on the reduction of the Apnea-Hypopnea Index (AHI), the Oxygen Desaturation Index (ODI), the minimal peripheric oxygen saturation (SpO2min) and in the results of the Epworth Sleepiness Scale (ESS). Study Design: retrospective cohort study Material and methods: Clinical processes of patients diagnosed with OSAS who had complete clinical assessment with polysomnography, drug induced sleep endoscopy (DISE) and Computed Tomography (CT) of the paranasal sinuses and pharynx, were analyzed. Those who underwent isolated nasal surgery as a therapeutic procedure and performed a control polysomnography were selected. The following variables, before and after the intervention, were submitted to statistical analysis: AHI, ODI, SpO2min, ESS result and weight. Results: Of the fifteen patients, mostly male (13/86.6%), there was a statistically significantimprovement of the AHI (p=0.019), ODI (p=0.008) and ESS (p=0,015) after surgery. There was no statistically significant association between SpO2min (p=0.089) or weight (p=0.862) before and after surgery. Conclusions: Isolated nasal surgery can be effective in OSAS, in selected patients.

Conclusions: Isolated nasal surgery can be effective in OSAS, in selected patients.Objectivo: Avaliar os resultados da cirurgia nasal isolada no tratamento da Síndrome da Apneia Obstrutiva do Sono (SAOS), através do impacto na redução do índice de Apneia-Hipopneia (IAH), do índice de dessaturação de oxigénio (ODI), da saturação periférica oxigénio mínima (SpO2min) e dos resultados da escala de sonolência de Epworth (ESE). Desenho do Estudo: estudo coorte retrospetivo Material e métodos: Foram consultados os processos clínicos dos doentes com diagnóstico de SAOS, que realizaram avaliação com estudo poligráfico do sono, estudo endoscópico da via aérea superior sob indução farmacológica do sono – Drug Induced Sleep Endoscopy (DISE) e Tomografia Computorizada (TC) de seios perinasais e faringe. Foram selecionados aqueles submetidos a cirurgia nasal como procedimento isolado com objetivo terapêutico e que realizaram estudo poligráfico do sono de controlo após a mesma. Pré e pós-operatoriamente, foram analisadas as seguintes variáveis: IAH, ODI, SpO2min, resultado da ESE e peso. Resultados: Foram incluídos 15 doentes, maioritariamente do sexo masculino (13/86,6%). Após a cirurgia, verificou-se uma melhoria estatisticamente significativa do IAH (p=0.019), do ODI (p=0,008) e da ESE (p=0,015). Não se verificou associação estatisticamente significativa entre a SpO2min (p=0,089) e o peso antes e após a cirurgia (p=0.862).
