Although the AMS800® and the AdVance XP® are two of the most known surgical solutions to treat this problem, literature about deciding factors to determine which is the best option for each patient is missing. This study’s goal was to fight this lack of knowledge. Methods and Materials: We performed a retrospective study in 46 patients who suffered from PPUI and were treated with the AMS800® and/or the AdVance XP®, between January 2013 and October 2017 in the Urological and Renal Transplantation Department of Coimbra Hospital and University Centre. The paper clinical processes, informatized information and the answers of the patients to our questionnaire of Satisfaction formed, all together, the database that was, posteriorly, analysed. The PPUI was divided in 4 categories, being defined by the number of Pads Per Day (PPD) used by the patients: dry, when 0 PPD or 1 for security, mild, when 2PPD, moderate, when 3 – 5 PPD and severe, when >5 PPD were used. Results: The mean usage of PPD 12 months after the AdVance XP® implantation was influenced by the staging of the prostate cancer, namely, by the N (p=0,041) and by previous procedures as Urethrotomy (p=0,011). This patients’ Satisfaction Degree was affected by previous Radiotherapy (RT) (p=0,048). The AMS800® was affected by a positive surgical margin after the tumour removal, showing that the patients who had this presented a higher mean of PPD at 12 months post-implantation than the ones who had a negative one (p=0,044).The PPD evolution at 3 and 12 months after the PPUI correction surgery was higher in the Artificial Urinary Sphincter (AUS) patients, comparing with the sling ones (p=0,011 andp=0,008, respectively). The Satisfaction Degree was higher depending on the dryness perceived by patients at 12 months, both with AMS 800® and AdVance XP® (p=0,020 and p<0,001, respectively). Conclusion: The AdVance XP® should be considered to treat more moderate PPUI in patients that were not submitted to Adjuvant RT, nor to urethral procedures, besides the RP. In the remaining cases the AMS 800® remains the gold standard treatment for this type of Stress Urinary Incontinence (SUI).Introdução: A Incontinência Urinária Pós-Prostatectomia (PPUI) acarreta um grande fardo socioeconómico, estando a sua prevalência a aumentar rapidamente acompanhada do também crescente número de Prostatectomia Radical (RP) realizadas hoje em dia. Apesar do AMS800® e do AdVance XP® serem duas das soluções cirúrgicas mais conhecidas para tratar este problema, há uma falta de literatura sobre quais são os fatores decisivos para determinar qual a deles é a melhor opção para cada paciente. O objetivo deste estudo foi colmatar esta falta de conhecimento.Métodos e Materiais: Nós realizamos um estudo retrospetivo em 46 pacientes que sofriam de PPUI e foram tratados com o AMS800® e/ou o AdVance XP®, entre janeiro de 2013 e outubro de 2017 no Serviço de Urologia e transplantação Renal do Centro Hospitalar e Universitário de Coimbra. Os processos clínicos em papel, a informação informatizada e as respostas dos pacientes a um questionário de Satisfação formaram, em conjunto, a base de dados que foi, posteriormente, analisada. A PPUI foi dividida em 4 categorias, sendo definida pelo número de Pensos Por Dia (PPD) usados pelos pacientes: seco, quando 0 PPD ou 1,por segurança, ligeira, quando 2 PPD, moderada, quando 3-5 PPD e severa quando >5 PPD eram usados.Resultados: A média de uso de PPD aos 12 meses após implantação do AdVance XP® foi influenciada pelo estadio do cancro da próstata, nomeadamente, pelo N (p=0,041) e por procedimentos prévios como a Uretrotomia (p=0,011). O Grau de Satisfação destes pacientes foi afetado por Radioterapia (RT) prévia (p=0,048). O AMS800® foi afetado por uma margem cirúrgica positiva aquando da exérese do tumor, mostrando que os pacientes que a tinham,apresentavam uma média de uso de PPD aos 12 meses após a implantação superior aos que tinham uma margem negativa (p=0,044). A evolução dos PPD aos 3 e 12 meses após a cirurgia de correção da PPUI foi superior nos pacientes que colocaram o AUS,comparativamente aos que colocaram a fita sub-uretral (p=0,011 e p=0,008, respetivamente).O Grau de Satisfação foi superior dependendo do grau de continência percebido pelos pacientes aos 12 meses, quer com o AMS800®, quer com o AdVance XP® (p=0,020 ep<0,001, respetivamente).Conclusão: O AdVance XP® deve ser considerado para tratar PPUI mais moderadas, em pacientes que não foram submetidos a RT Adjuvante, nem a procedimentos uretrais, além da RP.
