ADVERSE REACTIONS TO ANTIMICROBIAL AGENTS IN INTERNAL MEDICINE AND ORTHOPEDIC SERVICES. JOSINA MACHEL HOSPITAL, 2014

Resumo: | A descriptive, prospective study was conducted to characterize the incidence and type of Adverse Drug Reactions (ADR) to antimicrobial agents in patients hospitalized in internal medicine and Orthopedic services at “Josina Machel” Central Hospital, in Luanda, in the period from January to February 2014 . The occurrence of adverse drug reactions was assessed by daily review of the clinical history of the patients with active search for potentially adverse effects associated with prescription antimicrobial agents.Of a total of 206 hospitalized patients, 21 were affected by ADR, corresponding to an incidence rate of 10.2%. The incidence was significantly higher in the internal medicine service, occurring in 11.7% of 137 patients admitted, while in the orthopedic service the incidence was 7.2% (5/69).The highest incidence of ADR was recorded in patients aged 30-39 years in 10 patients (4.9%), and in the female gender (7.8%).The most common clinical manifestations of ADR were rash (17.2%), followed by headache, pruritus and nausea and vomiting (13.8%).The antimicrobials most frequently associated with the occurrence of ADR were the antimalarial, related to more than half the cases. Among the antibacterial agents cephalosporins were associated to five cases of ADR (23.8%).Most ADR were classified according to severity as mild (52.4%) and as probable (57.1%) regarding the attribution of causality. Given the growing impact of the occurrence of ADR we recommend pursuing this research, in order to further deepen this problem. |

Realizou-se um estudo descritivo, prospetivo, para caracterizar a incidência e tipo de reações adversas a medicamentos (RAM) antimicrobianos em doentes internados nos serviços de ortopedia e de medicina do Hospital Central Josina Machel, em Luanda, no período de janeiro a fevereiro de 2014. A ocorrência de RAM foi avaliada através da revisão diária da história clínica dos doentes, com pesquisa ativa de efeitos adversos potencialmente associados aos medicamentos antimicrobianos prescritos. Do total de 206 doentes internados, 21 foram afetados por RAM, correspondendo a uma taxa de incidência de 10,2 por cento. A incidência foi significativamente mais elevada no serviço de medicina, tendo ocorrido em 11,7 por cento dos 137 doentes internados, enquanto no serviço de ortopedia foi de 7,2 por cento (5/69). A maior incidência de RAM registou-se nos doentes de idade entre os 30 e os 39 anos, com dez doentes (4,9 por cento), e no género feminino, com 7,8 por cento.As manifestações clínicas das RAM mais frequentes foram o rash cutâneo, em 17,2 por cento dos afetados, seguido de cefaleias, prurido e náuseas e vómitos (13,8 por cento). Os antimicrobianos mais frequentemente associados à ocorrência de RAM foram os antipalúdicos, imputados a mais de metade dos casos. Entre os antibacterianos destacam-se as cefalosporinas, associadas a cinco casos de RAM (23,8 por cento).A maioria das RAM foi classificada, quanto à severidade, como moderada (52,4 por cento) e, como prováveis (57,1 por cento), relativamente à imputação de causalidade.
