Resumo: Choledochal cysts are anatomical variations of the intra or extrahepatic biliary tree. Reported frequency rates range from 1 case per 100.000 to 1 case per 2 million with a female-to-male ratio in the range of 4:1. We report a case of a 22-year-old male, with right upper quadrant pain, carrying a gastroduodenoscopy compatible with antral gastropathy and duodenal procidence by external compression. The computed tomography (CT) scan showed a choledocal cyst with microlithiasis inside, compressing the second portion of the duodenum. The patient performed an endoscopic retrograde cholangiopancreatography (ERCP) and a cholangio-MR (MRCP), that confirmed the diagnosis. Surgery included cholecystectomy and complete excision of the cyst with sphincteroplasty. No intra or postoperative complications were observed. Trata-se de uma patologia pouco frequente com uma prevalência de 1/100.000 a 1/2.000.000 com um ratio entre sexo feminino e sexo masculino de 4:1. Apresentamos o caso clínico de um doente de 22 anos, sexo masculino, referenciado à consulta de Cirurgia por dor abdominal a nível do hipocôndrio direito, portador de endoscopia digestiva alta (EDA) que evidenciava gastropatia antral e procidência duodenal por provável compressão extrínseca. Realizou tomografia computorizada (TC) abdominal que revelou quisto do colédoco, com microlitíase no seu interior, que moldava a face interna da 2a porção duodenal. Realizou colangiopancreatografia retrógada endoscópica (CPRE) que evidenciou quisto do confluente bilio-duodenal e litíase múltipla intra-quística. Realizou ainda colangio-ressonância magnética (CPRM) cujo resultado confirmou o diagnóstico. Foi submetido a colecistectomia com excisão do quisto e esfincteroplastia, sem intercorrências intra ou pós-operatórias.
