Desta forma, a ambulatorização deste tipo de cirurgias, sobretudo das endourológicas, tem tido uma evolução muito favorável, com a diminuição dos custos gerais do procedimento e menos complicações pós-operatórias para os pacientes. No entanto, assiste-se ainda ao internamento de pacientes elegíveis para ambulatorização, não existindo estudos focados no impacto de variáveis intra e extra paciente na ambulatorização e/ou internamento destes pacientes. Objectivos: outcome 1 - definir a taxa de ambulatorização total e da taxa de ambulatorização entre doentes elegíveis para tratamento ambulatório; outcome 2 - correlacionar variáveis intra e extrapaciente dos pacientes submetidos a cirurgia endourológica com a variável ambulatório/internamento; outcome 3 - correlacionar as variáveis intra e extrapaciente dos pacientes submetidos ao tratamento cirúrgico da litíase renal com o tempo de internamento. Metodologia: estudo retrospectivo dos doentes com doença litiásica renal submetidos a tratamento cirúrgico no Hospital Santa Maria entre 1 de Janeiro de 2017 e 31 de Dezembro de 2017. Foi definida a taxa de ambulatorização geral e a taxa de ambulatorização em doentes elegíveis para cirurgia de ambulatório; foi feita uma análise univariada entre covariáveis intra e extra paciente e a variável binária ambulatorização/internamento; foi feita uma análise univariada para as variáveis categóricas, e regressão linear e correlação para a variável contínua, entre as covariáveis e o tempo médio de internamento. Resultado: outcome 1 – taxa de ambulatorização geral de 49,50% na URS, 20,65% na RIRS flexível e 33,33% na RIRS semirígida; taxa de internamento de doentes elegíveis para ambulatorização de 27,46% na URS, 28,77% na RIRS flexível e 33,33% na RIRS semirrígida. Outcome 2 – associação estatística significativa das variáveis sexo (p-value=0,0007), stent pré-operatório (p-value<0,0001), CSD (p-value=0,0006), cálculos múltiplos (p-value=0,0005), localização do cálculo (p-value=0,0002), drenagem pós-operatória (p-value<0,0001), tempo operatório (p-value<0,0001) e febre pós-operatória (p-value=0,0007) com a ambulatorização vs internamento. Outcome 3 - associação estatística significativa das variáveis sexo (p-value=0,0016), ITU prévia (p-value<0,0001), CSD (p-value=0,0454), cálculos múltiplos (p-value=0,0119), cálculos coraliformes (p-value=0,012), drenagem pós-operatória (p-value=0,0213), febre pós-operatória (p-value=0,0042) e tempo operatório (r2=0,1136). Conclusões: A taxa de internamento nos doentes elegíveis para ambulatorização é de cerca de 28,47%, sendo que o sexo feminino, a existência de stent pré-operatório, o CSD ≥2cm do cálculo, a existência de cálculos múltiplos, a localização mais proximal do cálculo, a drenagem pós-operatória, o tempo operatório mais longo e a existência de febre pós-operatória parecem ser factores preditivos do internamento dos pacientes; o sexo feminino, a existência de ITU prévia, o CSD ≥2cm do cálculo, a existência de cálculos múltiplos, a existência de cálculo coraliforme, a inexistência de drenagem pós-operatória, a existência de febre pós-operatória e o tempo operatório mais longo parecem ser factores preditivos do aumento do tempo de internamento.Introduction: In the last decades, there has been a rapid development of the surgical treatment of kidney stones, with which came a significant increase in the post-surgical SFR (stone-free rate) and a decrease of the post-operation complication rate. Therefore, with less overall cost and less post-operation complications, the ambulatory surgical setting of treatment of urolithiasis, especially in endourologic treatment, has been on a steady rise. However, patients without comorbidities and with all the criteria for an outpatient treatment are still being hospitalized, having no studies been yet performed to compare the impact of variables patient dependent and patient non-dependent in the decision of hospitalization or discharge of these patients. Objectives: outcome 1 - establish an outpatient surgery rate in the total of patients and an outpatient surgery rate in the patients who met criteria for an outpatient surgery setting; outcome 2 – correlate variables there are patient dependent and patient non-dependent in endourological surgery with the binary variable inpatient/outpatient; outcome 3 - correlate variables there are patient dependent and patient non-dependent in the surgical treatment of urolithiasis with the duration of hospital stay. Materials and methodes: restrospective study carried on patients with urolithiasis who were surgically intervened in Hospital Santa Maria from the 1st of January 2017 until the 31st of December 2017. Establishment of an outpatient surgery rate in the total of patients and an outpatient surgery rate in the patients who met criteria for an outpatient surgery setting; univariate analysis carried between covariables patient dependent and patient non-dependent and the binary variable inpatient/outpatient; univariate analysis between categorical covariables and the average hospital stay, as well as linear regression plus correlation between interval variables and the average hospital stay. Results: outcome 1 – outpatient surgery rate in the total of patients of 49,50% in URS, 20,65% in flexible RIRS and 33,33% in semirrigid RIRS; outpatient surgery rate in the patients who met criteria for an outpatient surgery setting of 27,46% in URS, 28,77% in flexibel RIRS and 33,33% in semirrigid RIRS. Outcome 2 – statistical significant association between the variables gender (p-value=0,0007), preoperative stent (p-value<0,0001), CSD (p-value=0,0006), multiple stones (p-value=0,0005), stone location (p-value=0,0002), postoperative stenting (p-value<0,0001), operative time (p-value<0,0001) and postoperative fever (p-value=0,0007) with the hospitalization vs discharge variables. Outcome 3 - statistical significant association between the variables gender (p-value=0,0016), urinary tract infection before the procedure (p-value<0,0001), CSD (p-value=0,0454), multiple stones (p-value=0,0119), staghorn stones (p-value=0,012), postoperative stenting (p-value=0,0213), postoperative fever (p-value=0,0042) and operative time (r2=0,1136). Conclusions: The outpatient rate in the patients who met criteria for an outpatient surgery setting is of 28,47%, with the predictive factors of hospitalization being the female gender, having a preoperative stent, the CSD of the stone ≥ 2cm, having multiple stones, the more proximal location of the stone, postoperative stenting, longer operative time and having postoperative fever.
