Resumo: | Introduction: patient safety is a growing concern in healthcare management. The main objectives of our work were the detection of potentially avoidable errors in the treatment of patients with melanoma and search for solutions to improve patient safety. Material and methods: FMEA (Failure Mode and Effect Analysis) was used for analysis of the potentially avoidable errors in melanoma surgery. Results: Failure to attend surgery or post treatment follow up and wrong site surgery were the most potentially dangerous procedural errors found in melanoma treat- ment. An informatic application was used to improve communication between the laboratory and the clinician and to avoid both failures to deliver a positive result to the patient and loss of follow up. Conclusions: dermatologists, working together with other healthcare workers, must actively search for opportunities to improve patient safety du- ring melanoma treatment. KEYWORDS – Patient safety; Melanoma; Role of informatics. Introdução: A promoção da segurança do doente é cada vez mais essencial na organização dos pro- cedimentos na área da saúde. Os objetivos deste trabalho foram a deteção de potenciais falhas que podem ocorrer desde a marcação da excisão de um melanoma até às consultas de seguimento e propor/aplicar soluções. Material e métodos: Na análise das potenciais falhas na cirurgia do melanoma, recorremos a uma FMEA (Failure Mode and Effect Analysis). Resultados: As falhas encontradas que podem acarretar maior severidade para o doente foram a não comparência à cirurgia, excisão da lesão errada e não comparência à consulta pós cirurgia. Para prevenir a não entre- ga de resultados aos doentes e a perda assim da possibilidade de um tratamento eficaz ou de seguimento, recorreu-se a uma aplicação informática que alerta os clínicos quando há um resultado positivo para melanoma.Conclusões: É responsabilidade do dermatologista, em conjunto com outros profissionais, identificar as oportunidades e áreas onde a intervenção pode prevenir falhas no tratamento e seguimento dos doentes com melanoma.
