Para a correção do astigmatismo corneano e a obtenção da acuidade visual e independência de óculos desejadas, é necessário a aquisição pré-operatória precisa de parâmetros biométricos oculares e o cálculo preciso do poder esférico e cilíndrico da LIO a implantar. A acuidade visual é afetada pelo astigmatismo residual pós-cirúrgico, sendo que uma das fontes de erro mais importantes, durante o cálculo das LIO tóricas, é não considerar o astigmatismo da superfície posterior da córnea. Não ter em consideração a sua potência, resulta em sobrecorreção em olhos com astigmatismo a favor da regra (WTR) e subcorreção em olhos com astigmatismo contra a regra (ATR). Os tomógrafos baseados em tecnologia LED (light-emitting diode) fornecem medidas da curvatura posterior da córnea e astigmatismo. Num estudo recente, medidas da curvatura posterior da córnea usando tomógrafos baseados em tecnologia LED para a medição da curvatura anterior combinado com tecnologia Purkinje para a medição da curvatura posterior mostraram melhores resultados do que os tomógrafos baseados em tecnologia Scheimpflug no cálculo de LIOs. Atendendo a que os nomogramas e modelos matemáticos disponíveis atualmente, que consideram o astigmatismo posterior da córnea, são baseados em dados de tomógrafos que usam tecnologia Scheimpflug, surgiu a necessidade de caracterizar o astigmatismo corneano total e posterior utilizando uma nova tecnologia, introduzida em 2014, com o objetivo de melhorar os nomogramas e modelos matemáticos existentes. Devido à precisão limitada das tecnologias atuais e à falta de um método de eleição para a avaliação do astigmatismo corneano e particularmente do astigmatismo posterior da córnea, o conhecimento de parâmetros biométricos normativos é essencial para o cálculo das LIOs.Para além disso, todos os estudos que avaliaram o astigmatismo total e posterior da córnea basearam-se em tecnologia Scheimpflug, não tendo estes parâmetros sido avaliados por tecnologia LED numa amostra significativa de olhos normais. O objetivo deste estudo descritivo é caracterizar o astigmatismo total e posterior da córnea numa amostra significativa de olhos normais usando um tomógrafo baseado em tecnologia LED - Cassini (i-Optics, The Hague, The Netherlands) e avaliar a comparabilidade dos resultados obtidos com os tomógrafos baseados em tecnologia Scheimpflug e outros, suportando a consideração do astigmatismo corneano total na correção precisa do astigmatismo na cirurgia da catarata, melhorando a preditibilidade da correção e otimizando a qualidade visual dos doentes e consequentemente, a independência de óculos e a sua satisfação. Através da caracterização precisa do astigmatismo corneano total e posterior com esta nova tecnologia, tem-se como objetivo futuro melhorar os nomogramas e os modelos matemáticos existentes, baseando-se estes em dados biométricos mais completos e precisos.Cataract surgery is one of the oldest and most successful treatments in all of Medicine, having evolved into a refractive procedure with the use of toric intraocular lens (IOL). For the correction of corneal astigmatism and to obtain the desired visual acuity and spectacle independence, it is necessary to obtain precise preoperative ocular biometric parameters and precise calculation of the spherical and cylindrical power of the IOL to be implanted. Visual acuity is affected by the residual post-surgical astigmatism, and one of the most important sources of error, when calculating toric IOL, is not to consider astigmatism of the posterior corneal surface. Ignoring its power results in overcorrection in eyes with with-the-rule (WTR) astigmatism and under-correction in eyes with against-the-rule (ATR) astigmatism. Colour point-source light-emitting diodes (LED) tomography provides consistent measures of posterior corneal curvature and astigmatism. In a recent study, direct measurement of the anterior corneal surface with colour-LED tomography combined with Purkinje technology to measure the posterior corneal curvature showed better performance than a Scheimpflug camera in toric IOL calculation. Given that the nomograms and mathematical models currently available, which consider the posterior corneal astigmatism, are based on data from cameras using Scheimpflug technology, the need arose to characterize total and posterior corneal astigmatism using a new technology, introduced in 2014, to improve the existing nomograms and mathematical models. Accounting for the limited precision of current technologies and lack of a gold standard for evaluating corneal astigmatism and particularly the posterior corneal surface, knowledge of normative ocular biometric parameters is essential in IOL calculation. Furthermore, all the published studies that evaluated total and posterior corneal astigmatism used Scheimpflug technology, these parameters were not evaluated using colour-LED technology in a significant sample of normal eyes. The purpose of this study is to characterize the posterior and total corneal astigmatism in a large number of normal eyes using colour-LED tomography - Cassini (i-Optics, The Hague, The Netherlands) and evaluate the comparability with the data from Scheimpflug cameras and others, supporting the consideration of total corneal astigmatism in the precise correction of astigmatism in cataract surgery, improving the predictability of the correction and optimizing the visual quality of the patients and consequently, spectacle independence and satisfaction rates.
