Resumo: The relation between arterial hypertension and coronary artery disease is analysed. The importance of other concurrent risk factors of atherosclerosis and the less remarkable effi- cacy of antihypertensive treatment in what concerns coronary artery disease are both em­ phasised. Endothelial dysfunction related to several risk factors is mentioned as a key factor to functional and structural alterations of coronary arteries. Coronary pathophysiology in hypertensive heart disease is further discus­sed and several mechanisms responsible for ischemia, mainly subendocardiat in left ven­tricular hypertrophy, are described. Special emphasis is given to the potential importance of prevention and regression of left ventricu­lar hypertrophy and interstitial fibrosis in improving coronary reserve and ventricular function. Finally, the therapeutic measures available to potentially break the causal link between arterial hypertension and coronary artery disease are refereed.

Finally, the therapeutic measures available to potentially break the causal link between arterial hypertension and coronary artery disease are refereed.Faz-se a análise das interrelações entre hi­pertenção arterial (HTA) e doença coronária pondo-se em evidência a importância da coexistência de outros factores de risco para ate­ rosclerose e as explicações para o reduzido benefício obtido com a terapêutica anti-hiper­tensiva relativamente à doença coronária. Refere-se o papel da disfunção endotelial in­ duzida pelos vários factores de risco nas al­terações funcionais e estruturais das artéri­as coronárias. Aborda-se afisiopatologia coronária na car­diopatia hipertensiva e focam-se os diversos mecanismos responsáveis pela isquémia, habitualmente subendocárdica, no coração hi­pertrofiado do hipertenso. Chama-se a atenção para a importância que poderá ter a prevenção ou regressão da HVE e a diminuição da fibrose intersticial com a terapêutica anti-hipertensiva na melhoria da reserva coronária e prevenção da insuficiên­cia cardíaca.
