Non-pharmacological approach in the control of chemotherapy-induced nausea and vomiting

Resumo: | Nausea and vomiting are the most common side effects referred by patients receiving chemotherapy. These effects assume a negative impact in patient’s quality of life with consequences in their daily life activities, complications such as anorexia and nutritional deficits, increasing the morbidity with social and economical consequences, as well as patient’s survival status. The antiemetic often used as a prophylactic treatment for induced chemotherapy nausea and vomiting are the 5HT3 receptor antagonists (first and second generation); corticosteroids; dopamine receptor antagonists and NK1 receptor antagonists. Despite the pharmacological advances, nausea and vomiting remain a significant problem. The non pharmacological approach can be used as a complementary method. Acupuncture is the only alternative practice which efficacy was accepted by the international medical community. There are few trials that prove the acupuncture efficacy in the control of chemotherapy induced nausea and vomiting, but the ones that were published prove its advantage. Although this practice isn’t included in the Portuguese health system, the number of countries that use it as an alternative method is increasing. |

Estes assumem um impacto negativo na qualidade de vida dos doentes, na medida em que interferem nas actividades de vida diárias; induzem o aparecimento de complicações tais como anorexia e déficits nutricionais, aumentando o índice de morbilidade, com consequências a nível económico e social, e na sobrevida dos doentes. A terapêutica anti-emética utilizada actualmente como profilaxia nos tratamentos de quimioterapia inclui antagonistas dos receptores 5HT3 de 1ª e 2ª geração; corticosteróides; antagonistas dos receptores de dopamina e antagonistas dos receptores NK1. Contudo, apesar dos inúmeros avanços em termos de terapêuticas farmacológicas, as náuseas e vómitos constituem ainda um problema significativo para os doentes submetidos a quimioterapia. A abordagem não farmacológica pode ser uma metodologia complementar à abordagem farmacológica utilizada actualmente. A acupunctura é provavelmente a abordagem mais credível, e que reúne maior consenso no panorama da comunidade médica internacional. Existem poucos estudos que comprovem a eficácia da acupunctura no controlo das náuseas e vómitos induzidos pela quimioterapia. Contudo, os existentes são peremptórios em afirmar a sua eficácia, sendo a mesma aceite em inúmeros países e inserida nos seus sistemas de saúde como uma abordagem alternativa ou complementar.
