The effects of bleaching agents on the superficial structure of the enamel

Resumo: Introduction: About 30% of patients have a certain degree of insatisfaction with their tooth color, this leads to a high search for aesthetic bleaching treatments. This procedure is done with gels and others bleaching products. Growing evidence shows that the usage of these agents have an impact on the roughness and hardness of the enamel, which is independent of type of bleaching agent or its concentration. These agents are useful but their impact on the superficial surface of the enamel remains controversial. Objective: Enlightening the effects of bleaching agents on the superficial structure of the enamel. Materials and methods: Bibliographic review of articles between 2004-2023 on PubMed using the following keywords “enamel”, “bleaching agents”, “surface roughness”,” enamel surface roughness”. After a strict analysis with the submission of the inclusion and exclusion criteria, articles were selected. Results: The bibliographic research carried out 86 articles and 12 scientific articles were considered eligible. Ten analyzed the impact on the hardness and roughness of the enamel by agents based on peroxide derivates, 1 article the impact of pastes based on coal dust and 1 article associates the use of the bleaching process with brushing. The methods used to evaluate hardness and roughness were the Knoop method (n=2) and profilometer test (n=3). Discussion: The analysis showed that the use of agents based on peroxide derivates is associated with changes in enamel (n=8). Conclusion: The use of bleaching agents, has an impact on the enamel structure, causing changes in the surface layer and sublayers of the dental parts

O procedimento é realizado com recurso a géis contendo peróxido de hidrogénio ou outros agentes branqueadores. Evidência crescente mostra que a utilização destes agentes tem impacto na dureza e rugosidade do esmalte, independentemente do agente ou da concentração utilizada. Embora os produtos branqueadores sejam úteis, o impacto da sua utilização na estrutura superficial do esmalte é controverso. Objetivo: Identificar o impacto dos agentes branqueadores na estrutura superficial do esmalte. Materiais e métodos: Revisão bibliográfica de artigos indexados à PubMed entre 2004-2023, utilizando as palavras-chave “enamel”, “bleaching agents” e “surface roughness”. Definidos critérios de inclusão e exclusão para a seleção de artigos. Resultados: A pesquisa bibliográfica realizada retornou 86 artigos, tendo sido considerados elegíveis 12 artigos científicos. Dez analisaram o impacto na dureza e rugosidade do esmalte de agentes baseados em derivados de peróxido e 1 artigo o impacto de pastas baseadas em pó de carvão e 1 artigo associa a escovagem com o branqueamento. Os métodos utilizados para avaliação de dureza e rugosidade foram o método de Knoop (n=2) e teste de perfilómetro (n=3). Discussão: A análise evidenciou que a utilização de agentes com base em derivados de peróxido associa-se com alterações na dureza e rugosidade do esmalte (n=8).
