Assessment of quality of life of patients with Chronic Rhinosinusitis in a short and long-term after sinus surgery

Resumo: Objective: The purpose of this study was to assess the long-term quality of life after treatment with Functional Endoscopic Sinus Surgery (FESS) in patients with Chronic Rhinosinusitis (CRS) and possible existence of clinical features that could influence that result. Methods: In this retrospective study, were included 75 patients with CRS submitted to FESS, between May of 2013 and October of 2014 at a tertiary otorhinolaryngology center in Lisbon. Quality-of- life (QOL) outcomes were evaluated by performing SNOT-22 preoperatively and in 2 postoperative different times (average 11.2 and 89.2 months respectively). Patient and disease specific data that could influence quality of life were collected and statistical analysis was performed. Results: There was an improvement in the SNOT- 22 after surgical treatment both in the 1st (x= 19,43; σ= 17,60) and 2nd (x= 30,81; σ= 21,24) evaluation, compared to the preoperative period (x= 58,24; σ= 19,32) (p<0.001). A positive correlation was found between SNOT-22 (QOL) before surgery and their absolute short and long-term improvement (p<0.05).Any patient and disease-specific data were not found to influence the long-term QOL in patients with CRS after surgical treatment with FESS. Conclusion: Patients with a worse QOL and a high SNOT-22 result before surgery, will have better QOL in the long-term with CRS after FESS.

Conclusion: Patients with a worse QOL and a high SNOT-22 result before surgery, will have better QOL in the long-term with CRS after FESS.Objectivo: Este trabalho pretende avaliar a qualidade de vida em doentes com Rinossinusite Crónica (RSC), a longo prazo, após tratamento por Cirurgia Endoscópica Nasossinusal (CENS), e a existência de características clínicas que poderão influenciar esse resultado. Material e Métodos: Estudo retrospetivo de 75 doentes, submetidos a CENS por RSC, entre maio de 2013 e outubro de 2014 num serviço de otorrinolaringologia pertencente a um centro hospitalar terciário da região de Lisboa. Foi avaliada a qualidade de vida relativa à doença nasossinusal através do questionário Sinonasal Outcome Test (SNOT-22), aplicado no pré-operatório e em 2 períodos diferentes do pós-operatório (média (x) 11,2 e 89,2 meses respetivamente). Foram recolhidos dados específicos dos doentes e da doença, que pudessem influenciar a qualidade de vida, e realizada uma análise estatística dos mesmos. Resultados: Verificou-se uma melhoria dos resultados do SNOT-22 após tratamento cirúrgico, quer na 1a (x= 19,43; σ= 17,60), quer na 2a avaliação (x= 30,81; σ= 21,24), comparativamente ao pré- operatório (x= 58,24; σ= 19,32) (p<0.001). Foi encontrada uma correlação positiva entre o resultado do SNOT-22 (qualidade de vida) pré-operatório e a sua melhoria absoluta nos resultados a curto e a longo prazo (p<0.05). Nenhum dos outros dados específicos dos doentes e da doença se revelou influenciador da qualidade de vida a longo prazo em doentes com RSC após tratamento cirúrgico por CENS. Conclusão: Os doentes com RSC, identificados no pré-operatório com pior qualidade de vida, obtiveram melhores resultados na melhoria da mesma e sua manutenção a longo prazo.
