Resumo: It has been proven that the intervertebral disc herniations (IVDH) may suffer spontaneous regression of their contents over time. In an attempt to clarify the responsible mechanisms, numerous studies were made and some hypotheses were proposed. Among the possible theories, there is growing evidence to consider that the resorption of the IVDH is mediated by immunological mechanisms. The IVD is considered one of the human immunological privileged sites, being also the largest avascular organ in the human body. It is foreseen that the invasion of this immunologically privileged state that occurs during IVDH may expose the IVD to the immune system. Therefore, the IVD trigger an immunologically mediated response in order to reabsorb the herniated content. However, controversy remains, and as so the investigators perspective diverges between the possibility of a specific immune response associated with an inflammatory reaction and the possibility of occurring only an inflammatory reaction of the body-foreign type. The objective of this "monography" is to review the literature and systematize the possible mechanisms by which IVDH resorption occurs.
