Prevalence Of People With Diabetes Who Meet Therapeutic Targets In Different Risk Factors - The Reality Of A List

Resumo: | Introduction: The key role that HbA1c and blood pressure control plays in reducing microvascular complications in Diabetes Mellitus (DM) is currently known. The optimization of the blood pressure (BP) profile and dyslipidemia reduces the appearance of cardiovascular disease (CVD), one of the main causes of death in people with diabetes. Type of study: Analytical retrospective observational study. Methodology: Inclusion of patients with DM aged %E2%89%A520 years enrolled and attending a list, between January and October 2021. The BP values used resulted from the average of the last 2 records in SClinico%C2%AE, measured in consultation, with a calibrated and validated automatic sphygmomanometer. Clinical records: lipid profile, HbA1c and cardiovascular risk (CVR) calculated at the last visit. Results: Inclusion of 92 patients, 52.2% women and 47.8% men. Ages ranged between 53 and 94 years, with a mean age of 72 years. Only 57.6% had good glycemic control (A) and of the BP values (B), only 55.8% of individuals with high CVR and 44.9% of those with very high CVR had values within the target. There were greater control of CT (C) (90.7%), LDL (46.5%) and TRG (74.4%) in diabetics with high CVR, compared to individuals with very high CVR (75.5% ; 22.4% and 57.1% respectively). We found a higher prevalence of individuals meeting the ABC criteria, 25.0% (n=23), compared to the STARK study (2010), in which 18.8% met targets according to the American Diabetes Association (ADA). Defining current target values, ESC 2021 recommendations, only 16.3% (n=15) met the ABC criteria. Conclusion: Diabetic control rates are still below expectations, despite progress in the surveillance and control of these users in recent decades. There is still a need for greater investment in prevention strategies, control of CVR factors and earlier and more intensive pharmacological intervention to prevent and/or delay the complications of DM. |

A otimização do perfil tensional e da dislipidemia reduz o aparecimento de DCV, uma das principais causas de morte em diabéticos.Objetivo: Determinar a prevalência de utentes diabéticos que, no ano de 2021, atingiram alvos terapêuticos nos diferentes fatores de risco, de acordo com o seu RCVG (risco cardiovascular global). Tipo de estudo: Estudo observacional retrospetivo analítico.Metodologia: Inclusão de utentes com Diabetes Mellitus (DM) com ≥20 anos inscritos e frequentadores de uma lista, com consulta entre janeiro e outubro de 2021. Os valores de PA utilizados resultaram da média dos 2 últimos registos no SClínico®, medidos em consulta presencial, com esfigmomanómetro automático calibrado e validado. Registos clínicos: perfil lipídico, HbA1c e RCV calculado na última consulta.Resultados: Inclusão de 92 utentes dos quais 52,2% mulheres e 47,8% homens. As idades variaram entre 53 e 94 anos, sendo a média de idades 72 anos. Apenas 57,6% apresentava bom controlo glicémico (A) e dos valores de PA (B), apenas 55,8% dos indivíduos com RCV alto e 44,9% dos com RCV muito alto, tinham valores dentro do alvo. Verificou-se maior controlo do CT (C) (90,7%), LDL (46,5%) e TRG (74,4%) nos diabéticos com RCV alto, comparativamente aos indivíduos com RCV muito alto (75,5%; 22,4% e 57,1% respetivamente). Verificamos uma maior prevalência de indivíduos a cumprir os critérios ABC, 25,0% (n=23), comparativamente ao estudo STARK (2010), em que 18,8% cumpriam alvos de acordo com a American Diabetes Association(ADA). Definindo valores alvo atuais, recomendações da ESC 2021, apenas 16,3% (n=15) cumpriram os critérios ABC.Conclusão: As taxas de controle dos Diabéticos, continua aquém do esperado, apesar do progresso na vigilância e controle destes utentes nas últimas décadas.
