Turnover phenomenon in nursing: a scoping review

Resumo: Introduction: The phenomenon of Turnover is a current problem that needs to be analyzed in order to promote awareness and find strategies to promote retention. The present scoping review aims to map the scientific evidence regarding the main causes and consequences of the phenomenon of turnover in nurses working in health institutions. Systematic Review Method: Using the methodology of The Joanna Briggs Institute for scoping reviews (2015), research was carried out in the MEDLINE and CINAHL and then in three other databases. Articles written in English and Portuguese, from the European and North American context, between 2008 and 2018, were selected. Presentation of results: A total of 136 articles were identified and 38 considered eligible for analysis. Interpretation of Results: The potential causes of turnover are of three distinct orders: personal, relational and organizational. Regarding the consequences, some were identified directly affecting nurses and others patients. Even so, the main focus is still on the economic and financial costs felt by organizations. Conclusion: More studies are needed to address the role of nurse managers in preventing this phenomenon, as well as in promoting positive work environments.

A presente revisão scoping tem por objetivo mapear a evidência científica relativa às principais causas e consequências do fenómeno de turnover em enfermeiros a exercerem funções em instituições de saúde. Método de Revisão sistemática: Através da metodologia do The Joanna Briggs Institute para revisões scoping (2015), foi realizada uma pesquisa na MEDLINE e CINAHL e em três outras bases de dados. Foram selecionados artigos escritos em inglês e português, provenientes do contexto Europeu e Norte-Americano, no período compreendido entre 2008 e 2018. Apresentação de resultados: Um total de 136 artigos foram identificados e 38 considerados elegíveis para análise. Interpretação de Resultados: As causas potenciadoras de turnover são de três ordens distintas: pessoais, relacionais e organizacionais. As consequências afetam diretamente os enfermeiros e os utentes, ainda assim, o principal foco prende-se com os custos económicos e financeiros sentidos pelas organizações.
