Quality of life in failed back surgery syndrome

Resumo: Objectives: Assessment of self-perceived health status and quality of life in a sample of patients with failed back surgery syndrome (FBSS) and analysis of correlations between the bodily pain dimension of the SF-36, the remaining dimensions of the same instrument and patients clinical data. Methods: Cross-sectional study with convenience sampling that included 40 patients with FBSS observed in PMR consultation between January and October 2010. Clinical data were recorded and the Portuguese version of SF-36 was self-administered. SPSS 17.0 was used for statistical analysis. Results: The mean score in all subscales of the SF-36 was less than 50 points except in social function. There was a significant correlation coefficient between the bodily pain domain and physical functioning, mental health, social functioning and role emotional domains. There was a stronger statistical correlation in the last two cases. There was also a statistically significant correlation between bodily pain and number of surgeries that patients underwent. Conclusion: This sample of patients with FBSS presented SF-36 results that reflect a self-perception of decreased quality of life. Bodily pain, being related to other dimensions, has obvious implications in the quality of life of these patients, which suggests and reinforces the importance of describing the experience of pain as a multifactorial process. Keywords: Failed Back Surgery Syndrome; Pain; Quality of life.Objectivos: Avaliação da percepção do estado de saúde e qualidade de vida numa amostra de doentes com síndrome de cirurgia lombar falhada (SCLF) e análise de correlações entre a dimensão dor corporal do SF-36, as restantes dimensões do mesmo instrumento e dados clínicos dos doentes. Material e métodos: Estudo transversal, com amostra de conveniência constituída por 40 doentes com SCLF seguidos em consulta de MFR, entre janeiro e outubro de 2010. Foram recolhidos dados clínicos e solicitado o autopreenchimento da versão de língua portuguesa do SF-36. Realizada a análise estatística com o programa SPSS 17.0. Resultados: A média dos valores obtidos nos domínios do SF-36 foi inferior a 50 pontos, excepto no domínio da função social. Verificou-se correlação com significado estatístico entre o domínio dor corporal e os domínios desempenho físico, saúde mental, função social e desempenho emocional, sendo o grau de correlação estatisticamente mais forte nos dois últimos casos. Também se encontrou correlação estatisticamente significativa entre dor corporal e número de cirurgias a que os doentes foram submetidos. Conclusão: Nesta amostra de doentes com SCLF, as dimensões do SF-36 demonstram valores que traduzem uma percepção de qualidade de vida diminuída. A dor desempenha a placa giratória das restantes dimensões, com óbvias implicações na qualidade de vida destes doentes, o que reforça a importância da descrição da experiência de dor como um processo multifactorial.
