NUTCRACKER PHENOMENON IN AN HYPERTENSE WOMAN

Nutcracker syndrome is a clinical entity that derives from the compression of the left renal vein between the abdominal aorta and the superior mesenteric artery, which may manifest with abdominal or flank pain, hematuria and pelvic congestion syndrome. Arterial hypertension is a rare clinical manifestation of this entity.We describe the case of a 36-year-old woman with a history of arterial hypertension lasting 9 years, diagnosed during her first pregnancy, that was evaluated for exclusion of a secondary cause of hypertension. An alteration in renal vasculature anatomy compatible with Nutcracker phenomenon was identified, which leads us to consider the relationship between the patient’s arterial hypertension and this finding, since no other secondary causes of hypertension were recognized. Blood pressure control was achieved with medication adjustment. This case highlights the importance of diagnosing rare causes of hypertension, given its impact on treatment and morbidity.
