O rituximab (RTX), um anticorpo monoclonal quimérico anti-CD20, foi utilizado pela primeira vez no tratamento de cancros de células da linha B, mas o seu uso estendeu-se rapidamente a outras doenças imunológicas como artrite reumatoide, pênfigo vulgar e doenças renais.Objetivos: Pretendemos descrever a farmacocinética e a farmacodinâmica do RTX e sintetizar a evidência mais recente acerca da segurança e eficácia do RTX no tratamento de glomerulopatias incluindo a nefropatia membranosa (MN), vasculite associada ao anticorpo citoplasmático anti-neutrófilo (AAV), nefrite lúpica (LN), doença de lesões mínimas (MCD), glomeruloesclerose segmentar focal (FSGS), nefropatia por imunoglobulina A (IgAN), glomerulonefrite (GN) crioglobulinémica associada ao vírus da hepatite C (HCV) e outras GN.Métodos: Efetuámos uma revisão sistemática da evidência mais relevante disponível acerca da utilização do RTX em doenças glomerulares. Foram incluídos ensaios clínicos randomizados (RCT), meta-análises e normas de orientação clínica internacionais, assim como ensaios clínicos atualmente a decorrer.Conclusão: Foram realizados vários RCTs sobre a utilização de RTX, sendo a MN e a AAV as entidades clínicas em que o impacto desta evidência foi mais notória. As normas de orientação clínica para glomerulonefrites Kidney Disease: Improving Global Outcomes (KDIGO) 2020 recomendam o RTX como tratamento de primeira linha em pacientes com MN em risco moderado a alto para perda progressiva da função renal. Do mesmo modo, a combinação de RTX com glucocorticoides (GC) é uma possível terapêutica inicial de primeira linha em doentes com AAV. É também uma opção de primeira linha para a terapêutica de manutenção da remissão. Adicionalmente, o RTX pode ser utilizado como alternativa ou em adição à terapêutica inicial da LN classe III, IV ou V em doentes com doença ativa refratária, apesar de não existir evidência robusta para sustentar esta recomendação. No que toca às restantes GN, ainda não existem ensaios clínicos suficientes de modo a estabelecer o possível papel do RTX. Atualmente, existem vários estudos a decorrer que irão trazer informação valiosa no sentido de melhor estabelecer o papel do RTX no tratamento de GN.Background: In the past decades, biological therapies have revolutionized the treatment of the vast majority of immunological diseases. Rituximab (RTX), a chimeric anti-CD20 monoclonal antibody, was first used in the treatment of B-cell malignancies, but soon extended to other immunological diseases such as rheumatoid arthritis, pemphigus vulgaris and kidney disorders. The abrupt increase in the number of published articles on this drug has shed some light to its potential efficacy and safety in the field of glomerular disease. Objectives: We aim to describe RTX’s pharmacokinetics and pharmacodynamics and summarize the latest evidence on RTX’s efficacy and safety in the treatment of glomerulopathies including membranous nephropathy (MN), anti-neutrophil cytoplasmic antibody (ANCA) associated vasculitis (AAV), lupus nephritis (LN), minimal change disease (MCD), focal segmental glomerulosclerosis (FSGS), immunoglobulin A nephropathy (IgAN), hepatitis C virus (HCV) associated cryoglobulinemic glomerulonephritis (GN) and other GN.Methods: We conducted a systematic review of the most important data available on the use of RTX in glomerular diseases. We included published randomized clinical trials (RCT), meta-analysis and international guidelines, as well as registered on-going clinical trials.Conclusion: Several RCTs have been conducted to study RTX use, being MN and AAV the clinical entities where the impact of this evidence was most notorious. The Kidney Disease: Improving Global Outcomes (KDIGO) 2020 guidelines on GN now recommend RTX as a first line therapy in MN patients with moderate to high risk of progressive loss of kidney function. Likewise, RTX in combination with glucocorticoids (GC) stands as a possible first line initial treatment in patients with AAV. It is also a suitable first line option for maintenance therapy. Additionally, RTX may be used as an alternative or in addition to initial therapies in class III, IV or V in patients presenting active non-responding/refractory LN, although there is no robust evidence to support this recommendation. As for the other GN, we still lack sufficient clinical trials in order to establish the possible role of RTX.
