The Entrustable Professional Activities as part of Competency Assessment in Clinical Practice: an integrative literature review

Resumo: In 2005, Ten Cate advocates that competencies can be operationalized and evaluated by linking them to professional activities and introducing the concept of Entrustable Professional Activities (EPAs), defined as work units that allow a clinical supervisor to confirm that the student is ready to take responsibility for such activities. In order to understand the construction and application of APEs in a context of competency assessment and to launch new perspectives of their application, the question was raised: what is the scientific evidence on the assessment of competencies in clinical practice based on reliable professional activities (EPAs)? We conducted an integrative literature review, produced since 2005 in the databases - B-on, PubMed, Scopus, EBSCO and Web of Science, using as inclusion criteria: articles in Portuguese, Spanish or English; open-source full texts; articles that include the English expression “Entrustable professional activities”. From the search 145 articles were obtained, removing the duplicates. Applying the inclusion/ exclusion criteria the list was reduced to 123 articles. After the analysis of the full text, 30 references were extracted whose content was analyzed and categorized, and which point to the benefit of using the EPAs for curricular construction and adaptation to the assessment of competences. concluding that Olle Ten Cate’s EPA concept is replicable for different medical specialties and potentially replicable in other health professions. Para comprender la construcción y aplicación de EPAs en un contexto de evaluación de competencias y lanzar nuevas perspectivas de su aplicación, se planteó la pregunta: ¿cuál es la evidencia científica sobre la evaluación de competencias en la práctica clínica basada en actividades profesionales confiables (EPAs)? Realizamos una revisión integral de la literatura, producida desde 2005 en las bases de datos: B-on, PubMed, Scopus, EBSCO y Web of Science, utilizando como criterios de inclusión: artículos en portugués, español o inglés; textos completos de código abierto; artículos que incluyen la expresión en inglés “Actividades profesionales confiables”. De la búsqueda se obtuvieron 145 artículos, eliminando los duplicados. Aplicando los criterios de inclusión / exclusión, la lista se redujo a 123 artículos. Después del análisis de texto completo, se extrajeron 30 referencias cuyo contenido fue analizado y categorizado, y que apuntan al beneficio de usar el EPA para la construcción curricular y la adaptación a la evaluación de competencias. Se concluye que el concepto de APE de Olle Ten Cate es replicable para diferentes especialidades médicas y potencialmente replicable en otras profesiones de la salud.Em 2005, Ten Cate advoga que as competências podem ser operacionalizadas e avaliadas, ligando-as a atividades profissionais e introduzindo o conceito de atividades profissionais de confiança (Entrustable Professional Activities - EPAs), definidas como unidades de trabalho que permitem a um supervisor clínico confirmar que o estudante está pronto para assumir a responsabilidade por tais atividades. Com o objetivo de compreender a construção e aplicação das EPAs num contexto de avaliação de competências e lançar novas perspetivas da sua aplicação, colocou-se a questão: qual a evidência científica sobre a avaliação de competências em prática clínica baseada em atividades profissionais confiáveis (EPAs)? Conduziu-se uma revisão integrativa da literatura, produzida desde 2005 nas bases de dados – B-on, PubMed, Scopus, EBSCO e Web of Science, utilizando como critérios de inclusão: artigos em português, espanhol ou inglês; textos completos de acesso livre; ar tigos que incluam a expressão em inglês “Entrustable professional activities”. Da pesquisa obtiveram-se 145 artigos, removidos os duplicados. Aplicados os critérios de inclusão/exclusão a lista foi reduzida a 123 artigos. Após a análise ao texto completo, extraíram-se 30 referências cujo conteúdo foi analisado e categorizado, e que apontam para o benefício da utilização das EPA para a construção curricular e adaptação à avaliação de competências.
