CF assessment scales, namely the Mayo Fluctuations Composite Scale (MFCS), One Day Fluctuation Assessment Scale (OFAS) and The Dementia Cognitive Fluctuation Scale-Research Version (DCFS-R), are psychometric instruments useful for the characterization and differential diagnosis between DLB and Alzheimer's disease (AD) dementia.The aim of this study was to adapt and validate the MFCS, DCFS-R, and OFAS to the European-Portuguese population and to determine their psychometric properties in the discrimination between DLB and AD.For this, we selected a convenience sample consisting of patients with DLB and AD. After translation and adaptation, the Portuguese-European versions of the MFCS, OFAS, DCFS-R and NPI were applied by same member of the research team. The neuropsychiatric evaluation of these patients also included the Mini-Mental State Examination, Montreal Cognitive Assessment, Neuropsychiatric Inventory, and Unified Parkinson's Disease Rating Scale. Internal consistency, convergent and discriminant validities were assessed for each scale. After the scale’s adaptation, the validation studies were conducted within a sample of 40 patients (DLB: n=20/40), with no statistically significant differences between both dementia-forms in terms of demographic characteristics. Acceptable internal consistency was found for the OFAS and DCFS-R (Cronbach's alpha=0.777 and 0.737, respectively) and questionable for the MFCS (Cronbach's alpha=0.614). We found significant correlations between most subscores and the total score of the three scales, which correlated moderately with the NPI total score. Strong correlation between DCFS-R and MFCS was found (r= 0.81, p<0.001). Optimal diagnostic accuracy was found for DCFS-R (AUC: 0.731). Statistically significant differences between DLB and AD were only found for MFCS and DCFS (p=0.040; p=0.011).We concluded that the Portuguese-European versions of the scales presented are valid tests in determining CF in patients with DLB but some limitations in their psychometric properties suggest that their association may be more advantageous. Further validation studies are required, to evaluate their test-retest and interrater reliability properties, as well as its ability to detect CF in other forms of dementia.A Demência com Corpos de Lewy (DCoL) é a segunda causa de demência neurodegenerativa mais comum nos idosos e a flutuação cognitiva (FC) é uma das suas características distintivas. As escalas de avaliação de FC, nomeadamente, a Mayo Fluctuations Composite Scale (MFCS), One Day Fluctuation Assessment Scale (OFAS) e Dementia Cognitive Fluctuation Scale-Research Version (DCFS-R), são instrumentos psicométricos que poderão ser úteis na caracterização e diagnóstico diferencial entre a DCoL e a Doença de Alzheimer (DA). O objetivo deste estudo foi adaptar e validar as escalas MFCS, DCFS-R e OFAS à população Portuguesa-Europeia e determinar as suas capacidades psicométricas na discriminação entre DCoL e DA.Para isso, selecionámos uma amostra de conveniência constituída por pacientes com DCoL e DA. Após tradução e adaptação, as versões Portuguesas-Europeias da MFCS, OFAS e DCFS-R foram aplicadas pelo mesmo membro da equipa de investigação. A avaliação neuropsiquiátrica destes doentes incluiu ainda a aplicação do Mini-Mental State Examination, Montreal Cognitive Assessment, Inventário Neuropsiquiátrico e Unified Parkinson's Disease Rating Scale. A consistência interna e as validades convergente e discriminante foram avaliadas para cada escala. Depois da adaptação das escalas, os estudos de validação foram realizados em 40 pacientes, (DCoL: n=20/40) sem diferenças estatisticamente significativas entre as duas formas de demência em termos de características demográficas. Foi encontrada uma consistência interna aceitável para a OFAS e a DCFS-R (alfa de Cronbach=0.777 e 0.737, respetivamente) e questionável para a MFCS (alfa de Cronbach= 0,614). Encontrámos correlações significativas entre a maioria dos subpontos e a pontuação total das três escalas, as quais se correlacionaram moderadamente com a pontuação total do NPI. Foi encontrada uma forte correlação entre DCFS-R e MFCS (r= 0.81, p<0,001). A melhor precisão diagnóstica foi conseguida para a DCFS-R (AUC: 0.731). Foram encontradas diferenças estatisticamente significativas entre a DCoL e a DA apenas para MFCS e DCFS-R (p=0.040 e p=0.011, respetivamente).Concluímos que as versões Portuguesas-Europeias das escalas apresentadas são testes válidos na determinação da FC em pacientes com DCoL mas algumas limitações nas suas propriedades psicométricas sugerem que a sua associação possa ser mais vantajosa.
