Universal Health Coverage: an overview and its challenges regarding cancer. The Brazilian experience

Resumo: | Introduction: Universal Health Coverage (UHC), despite ambiguities that can be attributed to the term, leading to different interpretations and approaches by the health and governmental authorities around the world, continues to be currently the center of the discussions on the provision of affordable and quality health services. Particularly at a historical moment characterized by the economic recession, unemployment, the unhealthy lifestyle, the appearance of health calamities and the management of chronic noncommunicable diseases associated with the aging of the population, among other factors. Objectives: To investigate, through critical analysis, assumptions and scope related to the UHC proposal based on its concretion and contemporary context. In addition, exposing the implications of this conception in the right to health, to establish parallel and identify obstacles in the performance of the Single Brazilian Health System (SUS), with special attention to cancer. Materials and methods: The systematic bibliographic review was carried out in the search for scientific articles, in the Databases of Medline, Lilacs and Scielo, as well as official documents of international organizations, not published in journals. Results: They cannot be presented in numerical data, since it is a conjunctive analysis, in relation to health needs and determinants, where specific care models, service management, with limited cancer assistance participate. Conclusions: This is a multifactor problem that puts in perspective the need to deepen the discussion to guarantee the access and protection of populations in low-income countries, against catastrophic health spending. There is urgency to find solutions that can minimize or reverse the current scenario, investing in robust, inclusive and equitable public health systems. |

There is urgency to find solutions that can minimize or reverse the current scenario, investing in robust, inclusive and equitable public health systems.Introdução: A Cobertura Universal de Saúde (CUS), apesar das ambiguidades que possam ser atribuídas ao termo, levando a diferentes interpretações e abordagens por parte das autoridades sanitárias e governamentais do mundo todo, continua sendo na atualidade o centro dos debates sobre a prestação de serviços sanitários acessíveis e de qualidade, particularmente, num momento histórico caraterizado pela recessão econômica, desemprego, estilo de vida não saudável, surgimento de calamidades sanitárias, e manejo de doenças crônicas não transmissíveis associadas ao envelhecimento da população, entre outros fatores. Objetivos: Explorar, mediante análise crítica, os pressupostos e alcances relacionados à proposta da CUS com base na sua concretude e contexto contemporâneo. Igualmente, discorrer sobre as implicações desta concepção no direito à saúde, na procura de estabelecer paralelismos e identificar entraves no desempenho do Sistema Único de Saúde Brasileiro (SUS), com especial atenção ao câncer. Materiais e Métodos: Foi realizada revisão bibliográfica sistemática na procura de artigos científicos, nas bases de dados Medline, Lilacs e Scielo, assim como, documentos e separatas oficiais de organizações internacionais não publicadas em revistas. Resultados: Não podem ser apresentados em dados numéricos, pois trata-se de uma análise conjuntural, enquanto necessidades e determinantes em matéria de saúde, onde participam modelos de atenção específicos, gestão de serviços, financiamento e políticas públicas com baixa cobertura assistencial ao câncer, de limitado alcance social. Conclusões: Trata-se de um problema multifatorial que coloca em perspectiva a necessidade de aprofundar a discussão com o intuito de garantir acesso e proteção das populações nos países de baixa renda contra gastos catastróficos em saúde.
