Resumo: The authors describe the clinical case of a 32-year-old woman, 20 weeks pregnant, admitted  to our Medicine Department with nausea, vomiting, epigastric pain and thrombocytopaenia.Clinical and laboratorial evaluation led to the diagnosis of HELLP syndrome one week later. Given the development of acute liver failure and pre-eclampsia, an urgent therapeutic interruption of pregnancy was performed, allowing a rapid recovery of the clinical condition and laboratorial status. Some comments on differential diagnosis, clinical evolution and treatment approaches are mentioned. Using this case report the authors present short reviews on the HELLP syndrome (a clinical condition usually diagnosed in the Obstetric Department) and the anti-phospholipid syndrome. Considerations are drawn on the association of these two syndromes
